Effect Of Extended Nursing Intervention Based On WeChat Platform On The Quality Of Life In Children With Wilms’ Tumor After Operation

Objective:To evaluate postoperative quality of life in children with Wilms’ tumor,explore the effect of extended nursing based on WeChat platform,and to observe the influence of varied methods of extended nursing on the quality of life in children,in order to provide reference for improving postoperative quality of life and reducing the incidence of postoperative chemotherapy-related complications in children.Methods:A total of 90 inpatients with Wilms’ tumor in pediatric surgery department from March 2015 to November 2017 in a grade three hospital were randomly divided into control group(n=45)and intervention group(n=45)according to convenient sampling.The patients and the caregivers in the control group were given telephone follow-up after discharge,while the experimental group was additionally given extended nursing based on WeChat platform after discharge.The intervention duration was ranging from one day before discharge to 12 weeks after discharge.The scores of postoperative quality of daily life,and the quality of life during chemotherapy were evaluated separately before and after 3-month intervention.Meanwhile,the incidence rates of chemotherapy-related complications were monitored before and after intervention,caregivers’ nursing satisfaction and awareness of health knowledge were investigated.The questionnaires was used to investigate the caregivers in the research,and were collected by the investigators.The caregivers required to refill questionnaires if there were some missed blanks or logistical errors until the rechecks turn out to be correct.All data collected in the study were performed with the use of SPSS 21.0 software,and the continuous data was expressed by mean±standard deviation,categorical variables were expressed by frequency.Between-group comparison was made by t test,and comparisons applied independent-samples t test or chi-square test.A P value of less than 0.05 was considered to be indicate statistically significance for all analyses.Results:1.There were 87 patients and caregivers were recruited in the study,with 44 cases in the intervention group and 43 cases in the control group.No significant between-group differences were observed in baseline data,quality of daily life scores,and quality of life scores during chemotherapy before intervention(P>0.05).2.Before intervention,there was no significant between-group difference in total scores of daily life quality and scores of each dimension(P>0.05).After 12 weeks of discharge,the total scores of daily life quality and scores of each dimension in the intervention group were lower than intervention before(P<0.05),and the control group had a increase but a slight upward trend in above scores before intervention,but no significant differences were seen when compared to the intervention before(P<0.05).After 3 months of intervention,significant differences were found between the two groups(P<0.05).3.Before intervention,there were no significant between-group differences in total quality of life scores and scores in all dimensions(P>0.05).After 12 weeks of discharge,the intervention group had significant increased above scores during chemotherapy than intervention before(P<0.05),and the control group had increasing scores but a slight rise with time extension,but no significant differences were observed when compared to the intervention before(P>0.05).After 3-month intervention,the differences between the two groups were statistically significant(P<0.05).4.Before intervention,there were no significant difference in the incidence rates of chemotherapy-related complications between two groups(P>0.05).After 3-month intervention,the indicators above in two groups were lower(except for phlebitis,rash and hair loss in the control group)than intervention before.The intervention group had a significant reduction than intervention before,and was significantly lower than the control group,which showed significant differences between the two groups after intervention(P<0.05).5.After 3-month intervention,satisfactory,general satisfactory and unsatisfied caregivers were 34 cases,9 and 1,respectively,in the intervention group,and were 23,15,and 5 cases,respectively,in the control group.The intervention group had higher satisfaction rate than the control group(77.27% vs.53.49%,P<0.05)6.After intervention for 3 months,the average of health knowledge of two groups showed significant differences 23.71+2.59 in the intervention group vs.19.98+3.34 in the control group,P<0.05)Conclusion:Extended care based on WeChat platform can continuously focus on nursing requirement after discharge,provide targeted nursing and health education,and can significantly improve postoperative quality of daily life and quality of life during chemotherapy.Besides,extended care can improve awareness of related health knowledge of children and caregivers,enhance nursing ability of the caregivers,decrease the incidence rate of chemotherapy-related complications,and improve the caregivers’ nursing satisfaction,and promote early rehabilitation of children,so it is worthy of clinical promotion and practice.
