ObjectiveThere are not many cases reported of HVPG cases in China.This paper summarizes and analyzes the clinical data of 554 patients with HVPG portal hypertension,and observe the role of HVPG in the diagnosis and prognosis of portal hypertension in the domestic patients with the main cause of hepatitis B,and discuss the standardized operation of HVPG.MethodA retrospective analysis of 554 patients with HVPG from April 2010 to November 2017 in our department was conducted.Through the electronic medical record system,the patients' demography,etiology,clinical manifestations,signs,imaging,endoscopy,laboratory examination,HVPG measurement and other clinical data were collected,and the diagnosis,differential diagnosis and disease of HVPG in the portal hypertension were statistically analyzed.The application value and clinical situation of[1],such as emotion assessment and clinical treatment.Results1.The study included 554 patients with HVPG value of 14.98 ± 6.40 mmHg(1-44 mmHg);among them,sex and age were not related to HVPG;There was a correlation between etiology,Child-Pugh score,MELD score,decompensation,ascites,splenectomy,portal vein thrombosis,alanine aminotransferase,gluten transaminase,bilirubin,albumin,prothrombin time,urea nitrogen,hemoglobin,platelet and HVPG(The P values were<0.001,<0.001,<0.001,0.048,<0.001,<0.001,<0.001,0.009,<0.001,<0.001,<0.001,<0.001,0.007,0.037,0.013,<0.001,respectively).2.Using Child-Pugh score,portal thrombosis,hemoglobin and aspartate aminotransferase to establish HVPG model equation as follows:HVPG value =0.943*Child score--2.553*portal vein thrombosis-0.028 x HB+0.011*AST+11.130;portal vein thrombosis:there is one 1,no--0];3.According to the value of HVPG,the prevention and treatment plan for HVPG guided esophageal and gastric varices bleeding is summarized.Conclusion1.The hepatic venous pressure gradient can be used to assess the severity of portal hypertension,risk stratification,and predict the occurrence of complications,which is also the case in the patients with hepatitis B as the main cause of the disease.The pressure gradient of hepatic vein was correlated with Etiology,Child-Pugh score,MELD score,decompensation,ascites,splenectomy,portal vein thrombosis,alanine aminotransferase,aspartate aminotransferase,bilirubin,albumin,prothrombin time,urea nitrogen,hemoglobin and platelets.2.The determination of HVPG is influenced by many factors.Its standardized determination is of great significance.
