Objective To investigare changes of urine protein spectrum in patients with IgA nephropathy(IgAN)with hyperuricemia and analyze the relationship between them and clinical related indexes.Methods In 2016 1~12 months in the First Affiliated Hospital of Anhui Medical University,82 cases were diagnosed as IgA nephropathy patients,By analyzing the collected clinical data and pathological data,we explore the difference between the hyperuricemia patients(male >420?mol/L,female >350?mol/L blood uric acid)and normal uric acid group urinary protein spectrum including urinary albumin / creatinine,total protein / creatinine,total protein(U-TP),transferrin(TRF),retinol binding protein(RBP),N-acetyl beta-D-Glucosaminidase(NAG),albumin(Alb),Ig G,alpha 1 microglobulin(a1-MG),beta 2 microglobulin(?2-MG),creatinine(UCr),cystatin C(CYC)and fibrinogen degradation products(FDP).the 24 hours urine protein by immune turbidimetric method,urine total protein by baking coke phenolsulfonphthalein,NAG was treated with MPT-NAG,and creatinine was used in the detection of creatinine,and the residual turbidimetry was used.Results 1.In 82 cases of primary IgA nephropathy,the incidence of hyperuricemia in primary IgA nephropathy was 43.9%.2.Hyperuricemia group were higher than that in normal uric acid group(P < 0.05),the estimated GFR(e GFR)lower than the normal uric acid group(P < 0.05),and mean arterial pressure was higher than normal uric acid.There was no significant difference in age,sex,24-hour urinary protein,creatinine,cholesterol,triglyceride,BMI between the two groups(P>0.05).3.The distribution of renal pathological tissue in Oxford was characterized by: M1E0S1T0 was more common.Compared with normal uric acid group,the difference between M,E,and S was not statistically significant(P > 0.05)in the renal tubules.In terms of the type of interstitial lesions,the T2 in the hyperuricemia group was significantly higher than that in the normal uric acid group,while the T0 and T1 ratios were significantly reduced..4.Hyperuricemia group urinary protein spectrum in urinary alb/ cr,U-TP / cr were higher than the normal uric acid group(P < 0.05);and t U-TP,TRF,RBP,NAG,alb,Ig G,?1-MG??2-MG,CYC,and FDP and normal uric acid group,there were no significant differences(P > 0.05).4.In patients with IgA nephropathy and 24 hours urine protein and urine protein respectively in the spectrum of urinary alb/ cr,U-TP / cr,U-TP,TRF,RBP,NAG,alb,Ig G,?1-MG??2-MG showed positive correlation(P < 0.05).Conclusion 1.The incidence of hyperuricemia in primary IgA nephropathy is high.2.IgA nephropathy patients with hyperuricemia had significant differences in e GFR,urea nitrogen,and mean arterial pressure,suggesting that the hyperuricemia group is in a severe condition.3.There is difference in urine protein spectrum between patients with IgA nephropathy and those with hyperuricemia.The protein content of patients with hyperuricemia is higher.Monitoring the change of urine protein spectrum can evaluate the severity of the disease.
