Obcective:Investigate the prevalence of anxiety and depression in patients with COPD and explore the clinical efficacy of vitamin B1 in COPD patients with anxiety and depression.160 COPD patients who were hospitalized in respiratory medicine department of our hospital from January 2017 to December 2017 were selected.60 patients with COPD with anxiety and depression who were hospitalized in respiratory medicine department of our hospital in the same period and have passed the inclusion criteria and exclusion criteria were randomly divided into experimental group and control group.The experimental group was treated with the COPD standardized treatment with oral vitamin B1 tablets(20mg/time,3times/day).The control group was treated with COPD standardized treatment with placebo.HAMA and HAMD score and tumor necrosis factor-a concentration were measured before and four weeks after the treatment.Result:Among the 160 patients with COPD,the proportion of the anxiety score larger than 14 of the patients associated with anxiety mood was 50.63%.The proportion of the depression score larger than 20 of the patients associated with depression mood was 42.5%.And the proportion of the patients with anxiety and depression was 38.75%.2.Comparison of equilibrium between control group and experimental group:There was no significant difference in social demographic data between the two groups(P>0.05),and there was no significant difference in psychological and social factors and disease related characteristics between the two groups(P>0.05).The data of the two groups were balanced and comparable.3.The results showed that the HAMA score of 30 COPD patients was 19.20 ± 3.21 and the HAMD score was 26.26 ± 5.34 when the patients were admitted to hospital with HAMA and HAMD.The scores of HAMA and HAMD in 30 cases of the experimental group were 20.78 ± 4.85 and 25.21 ± 3.46 respectively.The concentration of TNF-a was 15.71 ±3.21pg/ml in the experimental group and 16.25 ± 3.20pg/ml in the control group.4.The score of HAMA in the experimental group decreased from 20.78 ± 4.85 to 8.60 ± 3.43 after the intervention of vitamin B1.The control group decreased from 19.20 ± 3.21 to 15.32 ± 3.25 before treatment.The HAMA score of the experimental group was compared with the control group after the intervention of vitamin B1.The difference was statistically significant(P<0.05).5.The score of HAMD in the experimental group decreased from 25.21 ± 3.46 to 11.23 ± 3.24 after the intervention of vitamin B1.The control group decreased from 26.25 ± 5.34 to 16.8 ± 3.30 before treatment.The HAMD score of the experimental group was compared with the control group after the intervention.The difference was statistically significant(P<0.05).6.The scores of TNF-a decreased from 15.71 ±3.21 pg/ml to 14.32 ± 3.78pg/ml in the experimental group after the intervention of vitamin B1.The control group decreased from 16.25 ± 3.20 pg/ml to 14.24 ± 3.43 PG/ml before treatment.It was suggested that the TNP-αin the experimental group was compared with that in the control group after intervention,and the difference was statistically significant(P>0.05).Conclusion:1.Anxiety-depression accounts for a high proportion of patients with COPD.2.Vitamin B1 can improve the anxiety and depression mood in patients with COPD combined with anxiety and depression.But Vitamin B1 has no effect on the change of TNF-a concentration in patients and needs a further study.
