| Objective: to explore the diagnostic and assessment values of galactose-3(gal-3)in heart failure. and further comparison with B-type natriuretic peptide(BNP)and left ventricular ejection fraction(LVEF)as the difference of the diagnostic and assessment values in heart failure.Methods: 63 patients with chronic heart failure were enrolled from 2016 to 2017 in Qingdao municipal hospital,collected another 30 patients without heart failure as a control group at the same time,which matching in age,sex.The selected patients’ sex and age.Collected smoking history,BNP,echocardiography and other clinically relevant basic treatment data of the patients,and the plasma gal-3 indicators of patients with venous blood were extracted.Comparison the differences and correlation of BNP,LVEF and Gal-3 between the two groups,and furtherly explore the diagnostic value of Gal-3,BNP in severe heart failure and in different classification of heart failure. The data were analyzed by SPSS 22.0 software.Measuring material data(x±s),T test was used to compare the measurement data between the two groups,and one-way anova was used for the comparison between multiple groups.The chi-square test was used to compare the counting data,Use Spearman correlation analysis,use ROC curve analysis the diagnosis value of Gal-3,BNP,LVEF in severe heart failure patients.The value of P<0.05 was regarded as statistically significant.Results: 1.The comparison between control group and heart failure group,the Gal-3,BNP,LVEF in severe heart failure group and the control group has significant difference(p < 0.001),in heart failure group,Gal-3(1519.89±811.02)ug/L,the BNP(636.45±645.00)pg/ml was significantly greater than the control group Gal-3ug/L(538.55±217.87),BNP(60.31±88.06)pg/ml,The LVEF(47.73±12.34)in the heart failure group was less than the control group LVEF(60.30 + 1.64)%;2.Spearman correlation analysis between gal-3,BNP and LVEF was conducted,and the correlation between gal-3 and BNP was significantly positive(r=0.567 p < 0.001);There was a significant negative correlation between Gal-3 and LVEF(r=-0.496 p <0.001).There was a significant negative correlation between BNP and LVEF(r=-0.655 p < 0.001);4.Heart failure was further divided into the ejection fraction retention group and the ejection fraction reduction group based on whether the ejection fraction was retained or not.Ejection fraction reserve group cases patients with the Gal-3(1383±775.17)ug/L,the BNP(398.86±339.70)pg/ml with reduced left entricular ejection fraction group cases with Gal-3(1671.78±821.88)ug/L with the BNP(917.91± 783.67)pg/m Gal-3 levels were significantly higher than control group(538.55± 217.87)ug/L with the BNP pg(60.31±88.06)/m(P < 0.001);Gal-3,the BNP for heart failure patients in ejection fraction reserve area under the ROC curve(AUC)in the difference was statistically difference(p < 0.001),Gal-3,the BNP’s ROC curve respectively covers an area of 0.881(95%CI: 0.786 0.976)sensitivity 81.8%,specificity of 90.0%,0.927(95% CI: 0.865 0.990)sensitivity 90.9%,specificity 83.3%;Gal-3,the BNP for heart failure patients in ejection fraction reducing area under the ROC curve(AUC)in the difference was statistically difference(p < 0.001),Gal-3,the BNP’s ROC curve respectively covers an area of 0.960(95%CI: 0.000 1.000)sensitivity 93.3%,specificity of 90.0%,0.938(95%CI: 0.829 1.000)sensitivity 93.3%,specificity 83.3%;.Conclusion: this study by discussing the correlation between gal-3 and heart failure,prove that Gal-3 was of great value in the diagnosis and evaluation of heart failure.BNP has the highest sensitivity value in diagnosing heart failure,and Gal-3 has the highest specificity value in diagnosing heart failure.The sensitivity and specificity of LVEF in the diagnosis of heart failure are not enough,the retention of left ventricular ejection fraction in partial heart failure was considered.Further explore Gal-3,the BNP for ejection fraction retained or not severe heart failure diagnostic specificity and sensitivity,found Gal-3,the BNP reserved for ejection fraction of severe heart failure and ejection fraction to reduce severe heart failure has high diagnostic value,for ejection fraction to reduce heart failure,Gal-3,the BNP diagnostic sensitivity similar to that of the Gal-3 diagnostic specificity is superior to the BNP. |
