Objective: To investigate the changes of BNP and LVEF in different degrees and the causes of heart failure in infants and their clinical applications.Methods: Retrospective analysis of clinical data of 290 patients with heart disease in infants showed that they were divided into 3 groups with no sign,mild and moderate-severe heart failure symptoms respectively according to the Ross classification;they were divided into 4 types of presence which were increased cardiac preload,increased cardiac afterload,decreased myocardial contractility,insufficienct ventricular diastolic filling with heart failure according to the etiology and hemodynamics;The clinical value of BNP and LVEF in the diagnosis of heart failure in infants was evaluated by comparing the changes of BNP concentration and LVEF value,using ROC curve analysis method.Results: BNP value had statistical difference in the classification of heart failure degree(P < 0.05),and LVEF value had statistical difference in the classification of etiology(P < 0.05).BNP was the highest in children with paroxysmal supraventricular tachycardia(713.03 pg/ml),followed by children with cardiomyopathy(620.22 pg/ml).The diagnostic threshold of BNP for heart failure was 87.25 pg/ml with sensitivity of 55.9% and specificity of 88.4%.The BNP threshold for distinguishing mild from moderate to severe heart failure was 168.5pg/ml,with sensitivity 92.3% and specificity 74.2%.The area under curve,sensitivity and specificity of LVEF in the diagnosis of heart failure were low.Conclusions: BNP can be used as a clinical monitoring index for the aggravation or alleviation of infantile heart failure,and it has more diagnostic value for distinguishing mild from moderate to severe heart failure.However,LVEF can differentiate between heart failure with decreased myocardial contractility and other types of heart failure,but it has limited value in judging the degree of heart failure and heart failure.
