Objective:To compare the clinical efficacy of Anchang decoction combined with mesalazine in the treatment of ulcerative colitis with spleen-kidney yang deficiency and to provide evidence base for the clinical treatment of ulcerative colitis through randomized controlled design and oral treatment with mesalazine.Methods:40 patients were selected according to the diagnostic criteria of spleen-kidney yang-type ulcerative colitis.According to random and controlled scientific research design principles, the patients were numbered in accordance with the order of treatment.According to the number, the computer software SPSS22.0 was used according to 1:1.The proportions were randomly divided into two groups: 20 cases in the treatment group(Anchang decoction combined with mesalazine group)and 20 cases in the control group(Mesalazine group). The histological scores of the intestinal mucosa before and after treatment were observed and recorded.Geboes Index, efficacy assessment, disease severity and activity(Modified Mayo score),scores for major symptom scores in Chinese medicine, secondary symptom scores for Chinese medicine, symptom evaluation, adverse events, and safety evaluation.Results:1.There was no statistically significant difference in gender, age, disease duration, and disease area between the treatment group and the control group before treatment,ie the baseline was consistent;the groups were comparable.2.Comparison of histological scores(Geboes index): Before treatment, the difference between the treatment group and the control group was not statistically significant(P> 0.05),comparable.After treatment, the histological scores of intestinal mucosa in both groups were decreased(P<0.05).However,the improvement of the histology of intestinal mucosa was better in the treatment group than in the control group(P<0.05).The difference was statistically significant.3.Comparison of efficacy evaluation: The total effective rate of the treatment group was 90.00%,and the total effective rate of the control group was 70.00%.The complete remission, marked effect, and total effective rate of the treatment group were better than the control group, and the number of invalid cases was less than the control group. There was a statistically significant difference between the two groups(P<0.05).4.UC disease severity and activity(modified Mayo score)comparison: Before treatment, the difference between the treatment group and the control group was not statistically significant(P> 0.05),comparable.After treatment, the severity and activity of UC in both groups were decreased(P<0.05),but the severity and activity of UC in the treatment group were significantly better than those in the control group(P<0.05).significance.5.Comparison of the main symptom scores of traditional Chinese medicine in ulcerative colitis: Before treatment, there was no significant difference between the treatment group and the control group(P>0.05),which was comparable. After treatment, the scores of the main symptoms of mucous bloody stool, tenesmus, diarrhea, abdominal pain, and abdominal distension in both groups were lower than those before treatment(P<0.05);however,the reduction in the scores of major symptoms in the treatment group was lower than that in the control group(P).<0.05),the difference was statistically significant.6.Comparison of TCM symptom scores of ulcerative colitis: Before treatment, the difference between the treatment group and the control group was not statistically significant(P>0.05),comparable;after treatment, the two groups of patients with cold limbs The scores of minor symptoms of waist, knees and weakness, fatigue, and poor appetite were lower than those before treatment(P<0.05),but the reduction in the scores of secondary symptoms in the treatment group was lower than that in the control group(P<0.05).),the difference was statistically significant.7.Comparison of TCM syndromes: The total effective rate in the treatment group was 95.00%,and the total effective rate in the control group was 70.00%.The treatment group was superior to the control group in the clinical remission, marked effect, and total effective rate of TCM symptom efficacy. The number was significantly less than the control group, and there was a statistically significant difference between the two groups(P<0.05).8.No adverse events occurred in the treatment group during the treatment period. In the control group,1 patient experienced stomach burning sensation and nausea at night after the oral administration of the drug on the 3rd day.At that time,no treatment was performed and the symptoms eased after 1 hour.No safety problems occurred in the treatment group during the treatment of the safety index evaluation. In the control group,1 case of Grade 2 safety problems occurred during the treatment period. Conclusion:Mesalazine alone and Anchang decoction combined with mesalazine have a curative effect in the treatment of ulcerative colitis of spleen-kidney yang deficiency, but the Anchang decoction combined with mesalazine group has a histological score(Geboes index)and efficacy from the intestinal mucosa.Assessment, severity of illness and activity(Mayo score),scores of major symptoms of traditional Chinese medicine, secondary symptom scores of Chinese medicine, evaluation of symptoms and symptoms of traditional Chinese medicine, adverse events, and safety evaluation were superior to mesalazine alone. Therefore,in the treatment of patients with spleen-kidney yang-type ulcerative colitis,Anchang decoction has a definite curative effect and can be more clinically applied and promoted.
