| Objective:To explore the clinical significance of Glasgow prognostic score(GPS)and inflammatory markers predicting the severity degree of cerebral hemorrhage in early stage.Methods:153 patients brain hemorrhage diagnosed as cerebral hemorrhage,on January 1,2016 to December 31,2017 in our hospital,were as cerebral hemorrhage;at the same time,41 healthy check-up person were as control group.According to the NIHSS,cerebral hemorrhage can be divided into light,moderate and severe.White Blood cells,Neutrophil,lymphocyte,platelet count,Neutrophil to lymphocyte ratio and Platelet to lymphocyte ratio in Peripheral Blood were collected.The GPS was calculated by the content of serum albumin and c-reactive protein.The relationship of GPS,NLR,PLR and the severity of cerebral hemorrhage were compared in two groups.ROC curve was used to analyze the correlation between the severity of cerebral hemorrhage and inflammatory markers(GPS,NLR,PLR etc).Results:1.The GPS scores,NLR and PLR in two group were statistically significant(P<0.01).2.The severity of intracerebral hemorrhage on GPS score,NLR,PLR in two group were statistically significant differences(P<0.05,P<0.01).3.Between the light and moderate group,the area under the ROC curve of the NLR was 0.779(95%CI:0.687-0.871,P<0.01),and the cut-off value was 3.700,and the sensitivity was 82.8%,and the specificity was 64.5%;the area under the ROC curve of NLR was 0.769(95%CI:0.694-0.843,P<0.01),the cut-off value was 3.295,the sensitivity was 67.8%,the specificity was 76.2%between the moderate and severe group;The NLR in the two groups was statistically significant for the severity of cerebral hemorrhage.4.The area under the ROC curve was 0.628 when severe cerebral hemorrhage was diagnosed with PLR,but the diagnostic value was weak.5.Between the light and moderate group,the area under the ROC curve of the GPS score was 0.754(95%CI:0.653-0.855,P<0.01),the cut-off value was 1,the sensitivity was 75.9%,the specificity was 74.2%;between the moderate and severe group,the area under the ROC curve of GPS score was 0.648(95%CI:0.560-0.736,P<0.01),the cut-off value was 1,the sensitivity was 47.8%,the specificity was 82.5%.The combine of leukocyte count,neutrophil count,CRP and NLR,the area under the ROC curve and GPS score was 0.853(95%CI:0.779-926,P<0.01),the sensitivity was 98.4%,96.0%between severe group and mild-to-moderate group;The combine of five makers were used to predict the severity of intracerebral hemorrhage,the area under the ROC curve was 0.798(95%CI:0.729-0.867,P<0.01),the sensitivity was 66,7%,the specificity was 82.5%.Conclusion:1.GPS and NLR can be used as economic,convenient and reproducible makers to predict the severity of cerebral hemorrhage.2.The combine of leukocyte count,neutrophil count,CRP,NLR,and GPS predicted the severity of cerebral hemorrhage priority to single maker.3.PLR was not applicable for the evaluation of severity of cerebral hemorrhage. |
