Analysis Of Clinical Features Of 588 Hepatitis C Related Primary Liver Cancer

Objective:to investigate the etiology and clinical features of Priamary Liver Cancer(Primary Liver cancer,PLC)' in the Yanbian Region;to investigate the relationship beteween the pathogenesis of PLC and hepatitis C virus(Hepatiies C virus HCV)infecion and the relationship between HCV-RNA load and AFP(a-fetoprotein)in HCV related PLC in this region.At last the incidence of hepatocellular carcinoma in this region and the relationship between genotyping and HCV-RNA load were statistically analyzed.Methods:Retrospective analysis was used to collect 1366 patients with PLC who were treated in the Afifiliated Hospital of Yanbian University from December to January 2008?2017.The gender,age,race and past drinking hisory of the patients were statistically analyzed,and the HCV-RNA load,HCV genotyping,AFP,liver function and Child-pugh classification and the primary clinical manifestations of liver cancer were statistically analyzed for 588 HCV related PLC patients.Define the relationship between HCV-RNA load of HCV related PLC patients and AFP and the relationship between genotyping and HCV-RNA load.Results:1.In 1366 patients with PLC,the ratio of male to female was 2.29:1,and the incidence of PLC was higher in males than in females.The incidence of PLC was higher in males at the age of 50 to 60,and in females at the age of 60 to70.Among them the ratio of men and women in HCV related PLC was 1.24:1.The total prevalence of male and female was basically the same,and there was no difference between men and women in all ages.2.The 588 patients with HCV related PLC includeed 454(76.95%)Chinese Korean patients,131(23.2%)Han nationality,and 3(0.85%)other ethnic groups.3.Among the 588 patients with HCV related PLC,there were 545(92.69%)cases of liver cirrhosis and 43(7.31%)patients without cirrhosis.4.The statistics of nearly 10 years of PLC showed a slow upward trend,and in 2016,the incidence of HCV related PLC was the highest.In 2017,the incidence of HCV related PLC was lower than in 2016,but it was still higher than the previous 8 years.5.HCV related PLC patients lacked typical symptoms,and the first symptoms of fatigue,anorexia is the most common(60.20%).6.There was a positive correlation between HCV-RNA load and AFP,and the expression of HCV-RNA increased with the increase of viral load.7.Different genotypes of viruses have different degree of viral replication activity.The replication of lb genotype HCV virus was the most active,and the viral load was 105?106 IU/ml;2a genotype HCV,followed by the virus load of 104?105 IU/ml.Conclusion:1.HCV related PLC patients lacked typical symptoms,and the first symptoms of fatigue,anorexia is the most common.2.There was a positive correlation between HCV-RNA load and AFP,and the expression of HCV-RNA increased with the increase of viral load.3.Different genotypes of viruses have different degree of viral replication activity.The replication of genotype lb HCV virus was the most active,and the viral load was significantly higher than that of other genotypes.
