Research About Antiretroviral Therapy Adherence And Quality Of Life Among Hiv-infected People In Ili Kazakh Autonmous Prefecture

Objective The purposes of this study was to investigate on antiretroviral therapy(ART)adherence and quality of life among human immunodeciency virus(HIV)infected people who were receiving ART at the town level in Ili Kazakh Autonomous Prefecture and to analyze the factors influencing ART and quality of life to document for improving antiretroviral therapy service for acquired immunodeficiency syndrome(AIDS)patients.Methods A cross-sectional survey was conducted by one-on-one interview in three ART services centers at the town level of Ili Kazakh Autonomous Prefecture.The subjects were 18 years old and elder,HIV-infected individuals who were receiving ART in the duration of the survey.After informed consent,interview was concucted following the questionnaire and and collect related information from system of AIDS follow-up managment.The content of this survey included demographic characteristics,health-risk behaviors,knowledges of ART,adherence of taking medicine of ART,depression and anxiety symptoms,quality of life and awareness of HIV infection within family as well.χ2-test,t-test,non-conditional multiple logistic regression and multivariate linear regression was used to examine influencing factors of ART adherence and quality of life.Results In this survey,a total of 443 AIDS patients were recruited,and 412 subjects were eventually included in the study.The age range of paticipants was 19~67 years old and 60.9%(251/412)of participants were male.The survey found that 75.0% of participants(309/412)were in good adherence to ART.The total score of quality of life was(54.16±7.71).The participants who had depression and anxiety symptoms accounted for 20.6%(85/412)and 14.3%(59/412)respectively.The univariate analysis showed that the ART adherence associated with gender,occupation,cigarette smoking,alcohol drinking,depression symptoms and informing family of HIV infection.The scores of quality of life were significant different between kinds of marital status,occupation,time of HIV infection diagonosed,numbers of CD4+T lymphocyte,adherence and knowledges of ART as well as symptoms of depression and anxiety.Multivariate logistic regression analyses showed that participants who’s occupation was worker were more likely to have bad adherence to ART compared with unemployed participants(OR=2.74,95%CI: 1.28~5.86).Participants who had alcohol drinking behavior or their status of HIV infection was disclosed to their family increased the risk of bad adherence to ART(OR=3.75,95%CI:1.89~7.43 and OR=3.16,95%CI:1.36~7.33 respectively).Participants who had depression symptoms were also more likely to had bad adhence to ART(OR=1.90,95%CI:1.09~3.32).The symptoms of depression and anxiety were associated with lower scores of quality of life(β=-0.39,P<0.001;β=-0.229,P<0.001).Patients with good adherence have higher quality of life(β=2.06,P=0.005).Of the participants,the more years of HIV infection who were diagnosed and the more knowledges related to ART who had were significantly increased their score of quality of life(β=0.34,P =0.001;β=0.64,P =0.014).Conclusions There were about 1/4 of participants had bad adherence to ART at town level in Ili.There was interaction between ART adherence and quality of life and mental health.Thus,we need to develop the strategies on improving knowledge related to ART,enhancing taking medication of ART on time ans well as providing sociopsychological support for ART.
