Objective:To study the expression of Tanreqing Injection on the TLRs and the downstream cytokines in peripheral blood mononuclear cells of patients with AECOPD.Methods:To select the Ruikang Hospital of Guangxi University of TCM affiliated to the Department of respiratory medicine and geriatrics AECOPD phlegm heat obstructing lung type of 60 inpatients.The patients were randomly divided into 30 cases in the treatment group and the control group.The control group was treated with conventional oxygen therapy,anti infection,antitussive and phlegm,antispasmodic and antiasthmatic treatment after admission.The treatment group was given Tanreqing Injection(Shanghai Kai Bao Pharmaceutical Co.,Ltd.),20ml plus 250ml 5%Glucose Injection or 0.9%Sodium Chloride Injection intravenously,1 times a day for10 days in the control group.Two groups of patients were received 5ml venous blood for first,eleventh days in the morning respectively.The percentage of TLR4 positive in peripheral blood mononuclear cells was detected by flow cytometry,and the levels of TNF-?,IL-8 and IL-17 in serum were detected by enzyme linked immunosorbent assay(ELISA).All of the 60patients were entered into the result analysis without falling off.Results:Two groups of patients with TLR4 positive percentage,TNF-?,IL-8,IL-17 were compared with pretherapy,and the difference was not statistically significant(P>0.05).After treatment,the treatment group total effective rate was 96.7%,and the control group total effective rate was 80%.The total efficiency of two groups had statistical difference(?~2=4.04,P<0.05);Comparing with TCM syndrome score of the two groups pretherapy,fever,the cough,the expectoration,the shortness of breath,the dry mouth and lung rale level were significantly decreased,and the difference was statistically significant(P<0.05),and the decrease of the treatment group was greater than that of the control group(P<0.05).When the two groups of patients post-treatment,the TLR4 positive percentage of peripheral blood,TNF-?,IL-8 and IL-17 of serum were decreased compared with pretherapy,the difference was statistically significant(P<0.05).Comparing between the two groups,the decrease degree of the treatment group was more obvious than that of the control group.(P<0.05).There was no serious adverse reaction in the two groups of patients during the treatment.Conclusions:Tanreqing injection can significantly reduce the TCM Syndromes score,positive percentage of TLR4,TNF-?,IL-8 and IL-17expression of AECOPD patients with lung phlegm heat syndrome;Tanreqing injection may inhibit the release of downstream cytokines through down-regulation of TLR4 levels,suggesting that TLRs signaling pathway may be involved in the abnormal activation of AECOPD caused inflammation factor.
