Study On Catastrophic Health Expenditures And Its Influence Factors Of Worker Households In Xinganling National Forest Region

ObjectiveThis study aims to evaluate the status of catastrophic health expenditures and explore the effect of family health expenditures on household consumption level through the investigation of the worker households in the Xinganling National Forest Areas.This study makes an analysis on the influence factors of the family health expenditure and provides some suggestions of policy to reduce catastrophic health expenditures and ensure the happiness of the worker families.MethodTaking the worker families in Xinganling national forest district as the object,using multi-stage stratified cluster random sampling,the form of household one-on-one interviews to collect the data of 790 families.The social demographic characteristics of families,the family economic status,the medical and heath condition,the social security conditions were displayed by descriptive statistics analysis.Investigating and analyzing the conditions of the catastrophic health expenditures by using the health economic index which involve in the incidence rate of the catastrophic health expenditures,average gap,relative gap,The single factor analysis and multi-factor statistical analysis were used to identify the influence factors that bring out the catastrophic health expenditures under the different defined standards.Results1.Catastrophic health paymentThe incidence rate of catastrophic health payment are 28.23%,15.32%,12.7 8%,8.73%respectively,under the definition of 15%,25%,30%,400.It is declined by 12.91%when the standard was raised from15%to 25%.The average gap of catastrophic health payment are 5.49%,3.43%,2.74%,1.69%.The average gap decreases gradually with the improvement of the definition standard.The relative gap of catastrophic health payment are 19.45%,22.39%,21.44%,19.36%.There is no obvious relationship between the reduce of relative gap and the improvement of standards.2.The influence factors of the catastrophic health paymentThe influence factors of the catastrophic health payment include:the scope of family population:the educational level of householder,population over 60 years of age,health status of family members,family economic level,hospitalization in a family,medical insurance breakage,minimum living standard guarantee family,be in debt,having deposits.the educational level of householder,health status of family members and family economic level are the protective factors of the catastrophic health payment.hospitalization in a family,population over 60 years of age,population under the age of five and be in debt are the risk factors of the catastrophic health payment.ConclusionThe incidence of catastrophic health expenditures of the worker households in the Xinganling National Forest Areas is generally at a medium level.Compared with international,the incidence rate of catastrophic health expenditure of the worker households in the Daxinganling national forest areas is higher than the western developed countries,lower than some Latin American countries.Compared with domestic,the incidence rate of catastrophic health expenditure of the worker households in the Daxinganling national forest areas is higher than the catastrophic health expenditure of urban residents in Hami region of Xinjiang,lower than the catastrophic health expenditure of residents in urban areas of Baoji City,Shanxi Province,is close to the incidence of disastrous health expenditure of urban residents in Shijiazhuang.The government should pay attention to the poor and increase the income of the poor through various channels,strengthen disease prevention and health care,strengthen the construction of grass-roots health service system,reduce the proportion of personal medical care and improve the social security system,increase educational input to improve the cultural and educational level of forest residents,Pay attention to the elderly and disadvantaged groups and promote the construction of the welfare system for the elderly to reduce catastrophic health expenditure and protect the health of forest residents.
