The Application Of Double Contrast-enhanced Ultrasonogrphy And Contrast-enhanced Computer Tomography For Preoperative TNM Staging Of Gastric Cancer

Objective To explore the clinical value of double contrast-enhanced ultrasonography(DCEUS)and Contrast-enhanced Computer Tomography(contrast-enhanced CT)for the preoperative TNM staging of patients with gastric cancer.Method The DCEUS and contrast-enhanced CT features of 65 cases of patients with gastric cancers were retrospective analyzed.Invasion depth(T staging),lymph node metastasis(N staging)and distant metastasis(M staging)were evaluated by DCEUS and contrast-enhanced CT.The results were compared with the postoperative pathological staging results.To evaluate the clinical application value of DCEUS.Results In 65 cases of patients with gastric cancer,there were 9 cases inT1 stage,10 cases in T2 stage,18 cases in T3 stage and 28 cases in T4 stage diagnosed by pathology;7 cases in T1 stage,14 cases in T2 stage,18 cases in T3 stage and 26 cases in T4 stage were diagnosed by DCEUS;4 cases in T0 stage,5cases in T1 stage,11 cases in T2 stage,19 cases in T3 stage and 26 cases in T4 stage were diagnosed by contrast-enhanced CT;The overall accuracy of DCEUS and contrast-enhanced CT in T staging were 76.92% and 75.38% respectively.There was no significant difference in accuracy of DCEUS and contrast-enhanced CT(P>0.05).There were 17 cases in N0 stage,48 cases in N+ stage diagnosed by pathology;23 cases in N0 stage,42 cases in N+ stage were diagnosed by DCEUS;12 cases in N0 stage,53 cases in N+ stage were diagnosed by contrast-enhanced CT.The overall accuracy of DCEUS and contrast-enhanced CT in N staging was 72.31% and 80.00% respectively.There was no significant difference in accuracy of N staging between DCEUS and contrast-enhanced CT(P>0.05).There were 59 cases in M0 stage,6 cases in M+ stage diagnosed by pathology;60 cases in M0 stage,5 cases in M+ stage were diagnosed by DCEUS;59 cases in M0 stage,6 cases in M+ stage were diagnosed by contrast-enhanced CT;The overall accuracy of DCEUS and contrast-enhanced CT in M staging were 98.46% and 100% respectively.Conclusions DCEUS has high accuracy in preoperative TNM staging of patients with gastric cancer.It might be useful for diagnosed the TNM staging of patients with gastric cancer before surgery.
