| Objective To explore the clinical effect of MB combined KI mediated PDT on oral candida albicans in AIDS patients and the optimal PDT treatment scheme.Methods 27 patients with AIDS combined with oral candidiasis were divided into PDT-A group,PDT-B group and fluconazole group,there are 9 persons in each group.In PDT-A group,200 uMMB and 20min of incubation;PDT-B group,400 uM MB and 5min of incubation.After incubation,300 mMKI was added,and 30min was irradiated with the same light source(635 nm)for one times a day continued for 3 days.Observing the oral lesion and clinical symptoms after 24h of PDT treatment,at the same time getting concentrated oral rinse culture from group A,B before and after PDT treatment to centrifuge,removing supernatant and making bacterium suspension.And then taking two 10 uL bacterium suspension.The number of candida albicans was calculated by the cell count plate with dilute bacterium suspension under the microscopeanother.Another 10 uL bacterium suspension was inoculated in CHROMagar candida color medium,comparing the changes of different colonies.Fluconazole group:daily oral administration of 100mg fluconazole,a total treatment of 7 days,observing lesions and clinical symptoms.Results(1)The survival rates of group A and group B were 80.36%and 77.22%after the first treatment respectively.The survival rate after the second treatment was 64.94%,61.96%,and the survival rate was 48.57%and 38.42%respectively after the third treatment.There were statistically significant differences(P<0.05)of the survival rate between treatment times in group A or B,and the survival rate and candida count of group A and group B had no statistically significant difference(P>0.05).(2)The efficiency and cure rate of group A,group B and fluconazole group were found a statistically significantdifference with x~2=1.286~a,P=0.526 andx~2=2.196~a,P=0.333 of the chi-square test(3)The incidence of adverse reactions of group A,group B and fluconazole had no statistically significant difference withx~2=5.041~a,P=0.082.Conclusion MB combined KI mediated PDT is effective in the treatment of oral candida albicans infections in AIDS patients.The appropriate concentration of photosensitizer or incubation time can make PDT effect equal to fluconazole.The accumulation of PDT can enhance the bactericidal effect.There was no significant difference in the incidence of adverse reactions between PDT and fluconazole. |
