Objective:To study the clinical efficacy of meniscus injury treated with Platelet-rich plasma(PRP)by different puncture points of knee joint,we observed drugs distribution different on the meniscus surface with different injection sites.Methods:1.From January 2016 to January 2018,18 human cadaveric knees were randomly divided into group A,group B and group C.6 cases in each group,group A(n=6,the patella middle-upper approach),group B(n=6,injected through the lateral joint line approach),group C(n=6,injected through the medial joint line approach).Knees of both groups were interfused with a 5%(mg:ml)methylene blue iohexol.Each specimen was then simulated a 5°-50°alternating walking for 2h in the Electro Force platform.Afterwards.Combined with C-arm X-ray,all the knee cavities were cut open to examine the range of methylene blue iohexol distribution.Based on the four-zoning classification law for the meniscus surface.the coverage area of methylene blue iohexol in each zone was scored and the characteristics of methylene blue iohexol distribution was depicted.2.Based on the experimental results,the second hospital affiliated to Dalian medical university 60 patients with the lateral meniscus injuries,their knees joint cavity were respectively injected with PRP by the different injection sites,then took the MRI and compared the clinical efficacy before and after treatment for 3 months.Results:1.The coverage area score comparison:the lateral and medial joint line approach(group B,C)> the patella middle-upper approach(group A);the lateral joint line approach(group B)> the medial joint line approach(group C)on the distribution of the lateral meniscus;the medial joint line approach(group C)> the lateral joint line approach(group B)on the distribution of the medial meniscus.2.Different injection sites of platelet rich plasma(PRP)had significant difference for the treatment of meniscus injury.Conclusions:1.Platelet rich plasma(PRP)has significant clinical efficacy for the knee meniscus injury.2.Different injection sites of platelet rich plasma(PRP)had significant effect on the treatment of the knee meniscus injury.3.The knees of the lateral meniscus surface injected with PRP had the broadest coverage area through the exterior joint line approach(group B);the knees of the medial meniscus surface injected with PRP had the broadest coverage area through the medial joint line approach(group C);Therefore,it is recommended that the lateral meniscus injury was injected with PRP through the lateral knee joint line,and the medial meniscus injury was injected with PRP through the medial knee joint line.For the knee meniscus injury(especially II degree)had an important guiding significance in the clinical application of conservative treatment.
