Background:With the development of orthopedic sports medicine,the continuous research on the meniscus and the increasing demands on people's motor function,the function of the meniscus has gradually gained attention.Platelet-rich plasma(PRP)has been widely used in recent years to repair meniscus injury.However,some in vivo experiments have found that the effect of platelet-rich plasma on the repair of meniscus injury is still controversial.The activation ratio and the frequency of intervention in the preparation of different platelet-rich plasma gels(PRG)may affect the repair efficacy of meniscus inj ury.Objective:To observe the effects of different activated proportional activator-activated platelet-rich plasma gels on the repair of meniscus injury and the effect of different treatment frequencies on the repair of meniscus injury in platelet-rich plasma.The ultimate goal is to observe whether the above two variables can cause differences in effects,and further derive the optimal activation mode and treatment frequency,which provides a certain reference for clinical application.Methods::First,a unified injury model was created in the adult white rabbit knee meniscus,and the knee splint was braked for two weeks.The left and right knees were treated side and untreated side,respectively.The treatment side was intra-articular injection of platelet-rich plasma gel(PRG)as an intervention,and there was no intervention on the untreated side.Phase I experiment:The treatment side of each group was divided into A,B,and C3 groups with the activation ratio of platelet-rich plasma gel(PRG),and 18 rabbits in each group.The ratio of platelet-rich plasma to activator was 3:1,5:1,and 7:1,respectively.At the 4th,8th,and 12th week after surgery,6 experimental rabbits were sacrificed in each group,and their knees and meniscus were taken for gross and histological observation to evaluate the ability of platelet-rich plasma to promote meniscus injury repair.Phase ? experiment:The treatment side of each group was divided into 4 groups according to the frequency of intervention.The frequency of intervention was only 1 time,once a week,once every 2 weeks,once every 4 weeks,12 rabbits in each group.Six rabbits were sacrificed in each group at the 8th and 12th week after operation.The bilateral meniscus was used for gross and histological observation.The platelet-rich plasma was used to promote the healing of meniscus inj ury.Results::Phase ? results:12 weeks after surgery,the results of meniscus healing were graded by collating the experimental data and histological observation.The healing grade of group C was better than that of group B and A.<0.05),there was no significant difference between the two groups(P>0.05).The healing of the treatment side of each group was better than that of the untreated side(P<0.001).The statistical analysis of the area of the unhealed gap as a percentage of the visual field was analyzed by statistical analysis of variance,P=0.009.According to the test level of a=0.05,the treatment sides of the groups A,B and C were considered to be different or not identical.Multiple comparisons between the treatment group of group A,group B and group C showed that the healing of group C was better than that of group B and group A,P<0.05,which was statistically significant.The healing of group B and group A was P>0.05,no statistical difference.The results of phase ? trials,12 weeks after surgery,there was no statistical difference between the groups(P>0.05).The area of the unhealed gap accounted for the percentage of the field of view is the measurement data.After statistical analysis,the treatment side of each group of A,B,C,and D,P=0.432,according to the test level of a=0.05,it is still not considered that the treatment side of each group There was a difference in the healing effect between the two,which was not statistically significant.However,visually,the healing grade of group C seemed to be better than that of group A,B,and D.The healing of each treatment side was better than that of the untreated side(P<0.001).Discussions::1.The healing results of the treated side with platelet-rich plasma are better than those of the untreated side,indicating that the effect of platelet-rich plasma on the repair of meniscus injury is positive.2.Platelet-rich plasma and activator activated by a 7:1 ratio of platelet-rich plasma gel can be used to promote the repair of meniscus white area damage may have a better effect.The effect of different platelet-rich plasma gel treatment frequency on the repair of meniscus white area in each group was not statistically significant.The significance of treatment frequency and cycle remains to be further explored and experimentally studied.4.In the histological observation and evaluation of this experiment,no obvious fibrocartilage cell regeneration was observed on the treatment side of each group,but it promoted fibrosis in the meniscus injury area and formed fibrous repair.5.This experiment is an animal experiment,rabbit tissue repair ability,metabolic speed and other indicators are different from humans.This experiment can only provide reference for platelet-rich plasma application methods and directions,and can not guide platelet-rich plasma in human tissue repair.The application of specific standardized treatments for humans still requires more experimental research to explore.
