Objective:Study results of nutrition risk screening,nutritional assessment,human body composition analysis and metabolic profile analysis of liver cirrhosis patients,and the relationship among each other.Explore suitable tools for nutritional status assessment in liver cirrhosis population.Methods:A total of 120 patients with cirrhosis and 20 healthy subjects were enrolled in this study.Respectively assess nutrition status of the objective via NRS-2002,SGA,BMI,ALB and TLC,body composition analysis,and metabolic profiles,and then research the correlation among those methods and the prognosis.Results:1.Prevalence of nutritional risk and malnutrition between cirrhosis patients from different etiology group showed no significant difference(p>0.05).2.Prevalence of nutritional risk and malnutrition diagnosed by SGA,BMI,corrected BMI,TLC,ALB,showed significant difference(P=0.000,P=0.037,P=0.000,P=0.002,P=0.000)among patients from different Child-Turcotte-Pugh group.3.Results of body composition,including PA,BCM,BFM,ECW,TBW and ECW / TBW showed no difference in different etiology groups(P=0.358,P=0.558,P=0.121,P=0.787,P= 0.398),while the result of SMM departed from each other(P = 0.337).4.PA,BCM,BFM,SCM and ECW/TBW were different among patients from different Child-Turcotte-Pugh groups(P = 0.000),while TBW showed no significant difference.5.The correlation between NRS-2002,corrected BMI,ALB,SGA,PA,BFM,ECW / TBW and Child-Turcotte-Pugh Score was moderate.BMI,TLC,BCM,SMM,TBW,ECW mildly correlated with Child-Turcotte-Pugh Score.6.Nutritional risk screening-2002,subjective global nutritional assessment,corrected BMI,ALB,PA,BFM,ECW / TBW moderately associated with bad events.BMI,TLC,BCM,SMM,and TBW were mildly correlated with bad events.7.Compared with non-nutritional risk group,the concentration of serum Hcy and Phe in nutritional risk group increased significantly(p <0.05);while concentration of Ala,Cit/Arg decreased significantly(p <0.05).Conclusion:Patients with cirrhosis commonly accompany nutritional risk and malnutrition.Macroscopic manifestations include human body composition(such as abnormal water distribution,body cell mass,skeletal muscle mass and body fat mass gradually decreasing with the progressing of cirrhosis).Microscopic manifest metabolic disorders such as disturbance of amino acids metabolome,and eventually leading to increased prevalence of severe complications in patients with cirrhosis,poor quality of life,increased risk of death and other adverse clinical outcomes.Prompt clinicians to routinely screen nutritional risk and select the appropriate nutrition screening / assessment tools for patients with cirrhosis;and initiate timely and targeted nutritional support and treatment,so as to achieve the purpose of improving clinical outcomes ultimately.
