Clinical Application Of Upper Eyelid Rejuvenation By Infrabrow Excision Blepharoplasty Of Biplanar Lift

Background:Upper eyelid relaxation is a degenerative change with age.It appears earlier in the face during the facial aging.At first,it was considered that the eyelid relaxation was characterized by losing skin elasticity,skin laxity,and increased wrinkles.People try to realize the upper eyelid rejuvenation by removing excess skin.With the deepening of the research,it is gradually realized that facial appearance is determined by the balance between different parts.With the occurrence of aging,the changes of the bone,muscle,fibrous tissue,skin and other support structures,such as the volume,shape,position,etc,break the balance between the parts.Due to the weakening of orbicularis oculi muscle,the relaxation and aging of the upper eyelid skin often accompany with the relaxation and sag of orbicularis oculi muscle.Therefore,the thinner subcutaneous tissue of the upper eyelid,the worse scalability orbicularis oculi muscle,the increasing of orbital fat capacity and the relaxation of orbital septum are all manifestations of aging and relaxation of the upper eyelid.There are many surgical methods to correct upper eyelid relaxation including: coronal incision,endoscopic eyebrow lifting,blepharoplasty,eyelid margin incision,and eyebrow incision for upper eyelid relaxation correction(including upper eyebrow incision,infrabrow excision,eyebrow excision),etc.These operations and Botox injection can improve the morphology of the upper eyelid and realize the upper eyelid rejuvenation,but each method has its advantages and disadvantages.It can't be widely accepted by the beauty seekers.In this paper,we discuss how to enact individualized surgical treatment according to different conditions.This method can correct the upper eyelid relaxation by one infrabrow hidden incision,biplanar lift,applied flexibly programmes.Finally it can realize the upper eyelid rejuvenation.Objective:To summarize the clinical experience of 52 cases on upper eyelid skinrelaxation,and to explore the clinical application and experience of upper eyelid rejuvenation by infrabrow excision blepharoplasty and biplanar lift.Methods:Collect the 52 beauty seekers' clinical data from 12,2015 to 1,2018.They were operated on by infrabrow excision blepharoplasty of biplanar lift to correct the lax upper eyelid skin in our hospital.They were all women between the ages of 35~65.The average age is 48.3 years old.They all had different degrees of upper eyelid skin laxity,double eyelid folds narrowed or disappeared and so on.Some had swollen upper eyelid.The extra orbital fat of the 30 cases was removed at the same time in the operation.The retro-orbicularis oculi fat(ROOF)of the other 22 cases was removed at the same time in the operation.According the upper eyelid shape and the personal requirements of beauty seekers,the infrabrow excision blepharoplasty of biplanar lift was used flexibly to realize the upper eyelid rejuvenation.The postoperative follow-up was observed at 1,3,6,and 12 months.Results:In the 52 cases,the upper eyelid shape was good,the incision healed well,the scar of the incision was hidden,no obvious scar hyperplasia and abnormal sensation occurred,no other complications such as upper eyelid dysraphism happened after the operation.Bilateral symmetry was good,and higher degree of satisfaction was obtained after operation.Conclusion:1.For the beauty seekers with upper eyelid skin laxity and the distance between eyebrows and eyes is nomal,it should be chosen to use the infrabrow excision blepharoplasty of biplanar lift to correct the upper eyelid relaxation.2.We should make the preoperative assessment well,and we should make a personalized design according to the upper eyelid shape and personal requirements of the beauty seekers.It can solve many problems by one infrabrow incision and realize the upper eyelid rejuvenation.3.It has many advantages to use the infrabrow excision blepharoplasty of biplanar lift to realize the upper eyelid rejuvenation,including postoperative fast recover,natural upper eyelid shape,lesser trauma,hidden incision imprint and less complications.It's an improvement and innovation in traditional and mature surgical methods.These advantages are worthy of clinical promotion with high levels of patient satisfaction.
