Objectives:Diabetes is a chronic metabolic disorder characterized by blood sugar levels increased.Its pathogenesis is based on insulin resistance and(or)the relative or absolute lack of insulin secretion,which can be caused by the combination of genetic and environmental factors.With the progress of the disease,a series of complications have seriously affected the health and quality of life of diabetic patients.Severe cases can even cause disability,death,and impose a huge burden on patients and society,which seriously threatens human life and health.At present,due to the limitations of medical standards,the diagnostic equipment for chronic complications of diabetes is not yet universal,so there is a certain lag in the diagnosis and treatment of chronic complications of type 2 diabetes.It is usually when patients have obvious symptoms that they pay attention,and the prognosis is poor,which seriously affects the quality of life of diabetic patients.Therefore,early assessment,early detection,and active prevention are particularly important.At present,T2 DM has become a major health burden worldwide.And with the development of the disease,diabetic patients often accompanied by abnormal thyroid function.Studies have found that diabetes and thyroid dysfunction often coexist.However,the correlation between chronic complications and thyroid hormones in patients with type 2 diabetes is not yet clear.At present,the diagnosis of type 2 diabetes and the detection of thyroid hormone levels have been popularized.It is simple and quick.Assuming that thyroid hormone levels can predict the prognosis of patients with type 2 diabetes,the shortcomings of current lag in diagnosis and treatment will be greatly improved.Studies have shown that thyroid hormone levels have a certain correlation with diabetic patients' blood glucose,blood lipids,and insulin resistance.The two interact and influence each other.The two interact and influence each other.The current study on the relationship between thyroid hormones and chronic complications in patients with diabetes has not been conclusive.Therefore,the purpose of this study was to investigate the relationship between thyroid hormone and chronic complications of diabetes in patients with type 2 diabetes,and its reliability as an indicator for the prediction of chronic complications of diabetes.Method:A total of 129 patients from hospitalized in Department of Endocrinology,China-Japan union Hospital of Jilin University,with simple type 2 diabetes were selected randomly from September 2015 to December 2015.The diagnostic criteria for type 2 diabetes were based on the 1999 World Health Organization(WHO)diagnostic criteria,Excluding type 1 diabetes,GDM,special types of diabetes and unknown type of diabetes;Exclude acute and chronic infections,rheumatic immune system diseases,hematological diseases,pituitary diseases,hyperglycemia caused by pancreatic space occupying disease;first disease is stress state caused by cardiovascular and cerebrovascular diseases and other causes,and fundus lesions caused by various causes,foot lesions;has been accompanied by chronic complications of diabetes,etc.From January 1,2016 to December 31,2017,21 months follow-up.If one patient gets one or more complications,he will not be following up.I will Collect general data and related laboratory tests including gender,age,height,weight,duration of diabetes,family history,BMI,fasting blood glucose(FPG),2h postprandial glucose(2h-PG)and fasting insulin FINS),2h postprandial insulin level(2h-PINS),lipids,and 24-hour microalbuminuria were measured to calculate the homeostasis index of insulin resistance(HOMA-IR).Thyroid hormone(TT3,TT4,FT3,FT4,TSH),fundus examination,peripheral neuropathy examination,lower extremity arteriovenous ultrasonography,cervical vascular ultrasound.Result:Univariate analysis showed that family history of patients,fasting blood glucose,TT3 and FT3 levels could affect the complication of patients with type 2 diabetes,the differences were statistically significant(P <0.05);multivariate Cox proportional hazards model results showed that patients with TT3 levels(RR = 1.567,95% confidence interval: 1.132-2.170,P = 0.001).The area under the curve of TT3 level under ROC curve was 0.786,which was higher than that of FT3,which was significantly higher than that of FT3 Sensitivity and specificity were 94.40%,58.10%.Conclusion:1.Thyroid hormones are associated with complications of type 2 diabetes mellitus;2.Total triiodothyronine is more meaningful for predicting the severity of chronic complications of type 2 diabetes.
