| Objective:Observation of clinical effects on the prevention of patients with Baofukang suppository in cervical cancer following radiation therapy suffering from vaginal adhesion,to discuss and study in detail compared with vaginal irrigation in principal、clinical treatment mechanism and results、adverse reactions,and clinical treatment widely.Object:From September in 2016 to March in 2018,199 female patients with cervical cancer after radiotherapy in vitro and intracavitary in radiotherapy department in the second hospital of Jilin university.6MV-X radiation and IMRT are used in radiotherapy in vitro and the total dose of pelvic radiation is PTV: 45-50 Gy,1.8Gy or 2Gy every time,once a day,five times a week.The dosage is increased by 60 Gy in the patients with pelvic lymph node positive.CTV include corpus uteri,cervix uterus,3 cm under the affected vagina,abdominal aorta,common iliac,internal iliac,external iliac,obturator lymphatic drainage areas.At the end of external irradiation therapy,high dose rate intracavitary radiotherapy is performed,once or twice a week.The patients who underwent the operation are treated by intracavitary brachytherapy twice,and its a single dose is 5Gy,total dose of vaginal stump reached 60 Gy.Patients who were not operated on are treated by intracavitary brachytherapy four to six times,and its a single dose is 6Gy,total dose of vaginal stump reached 80 Gy.The patients were randomly divided into control group(n=100)and observation group(n=99).Method:1.The control group: From the end of radiotherapy after hospital discharge,the vagina is rinsed every day and regular return to the hospital for review.The reviews include: vaginal cleanliness and(or)cytological examination of vagina at the end of the third month and the sixth month,high-risk HPV DNA test at the end of the sixth month,check the vagina suffering from vaginal adhesion or not with pelvic examination or vaginal colposcope at the end of the third month and the sixth month and the experimental results are analyzed and evaluated.2.The observation group: From the end of radiotherapy after hospital discharge,two boxes of Baofukang suppository are used every month and regular return to the hospital for review.The reviews include: vaginal cleanliness and(or)cytological examination of vagina at the end of the third month and the sixth month,high-risk HPV DNA test at the end of the sixth month,check the vagina suffering from vaginal adhesion or not with pelvic examination or vaginal colposcope at the end of the third month and the sixth month and the experimental results are analyzed and evaluated.Results:(1)Vagina adhesion: the end of the third month : In the control group,82 cases have no vagina adhesion,18 cases have different degrees of the vagina adhesion;in the observation group,89 cases have no vagina adhesion,10 cases have different degrees of the vagina adhesion.The above numerical differences were not statistically significant(P >0.05).The end of the sixth month : In the control group,71 cases have no vagina adhesion,29 cases have different degrees of the vagina adhesion;in the observation group,81 cases have no vagina adhesion,18 cases have different degrees of the vagina adhesion.The above numerical differences were not statistically significant(P >0.05).(2)Degree of vaginal cleanliness(Degree of Ⅰ score 1,degree of Ⅱ score 2,degree of Ⅲ score 3,degree of Ⅳ score 4):the end of the third month : In the control group,10 cases are degree of Ⅰ,48 cases are degree of Ⅱ,42 cases are degree of Ⅲ,0 case is degree of Ⅳ.in the observation group,49 cases are degree of Ⅰ,44 cases are degree of Ⅱ,6 cases are degree of Ⅲ,0 case is degree of Ⅳ.In the control group:2.32±0.65,In the observation group:1.57±0.61.The above numerical differences were statistically significant(P <0.05).the end of the sixth month : In the control group,6 cases are degree of Ⅰ,36 cases are degree of Ⅱ,58 cases are degree of Ⅲ,0 case is degree of Ⅳ,in the observation group,62 cases are degree of Ⅰ,29 cases are degree of Ⅱ,8 cases are degree of Ⅲ,0 case is degree of Ⅳ.in the control group:2.52±0.61,In the observation group:1.45±0.64.The above numerical differences were statistically significant(P <0.05).(3)The results of high-risk HPV:In the control group,24 cases were positive and 76 cases were negative,the overcast rate of HPV was 25%;In the observation group,40 cases were positive and 59 cases were negative,the overcast rate of HPV was 59.6%.The above numerical differences were statistically significant(P<0.05).Side reaction:1.In the control group,seven patients suffered vaginal bleeding,we tell them to operate gently and continue to observe,then the symptoms disappear.Tell them to continue to follow the doctor’s advice on vaginal irrigation.2.In the observation group,there was no adverse reaction.Conclusion:1.Compared with vaginal irrigation,there were no statistically significant on the prevention of patients with Baofukang suppository in cervical cancer after radiation therapy suffering from vaginal adhesion.2.Compared with vaginal irrigation,there were statistically significant on the improvement of vaginal cleanliness in patients with Baofukang suppository in cervical cancer after radiation therapy.3.Compared with vaginal irrigation,there were statistically significant on the treatment of reducing the risk of high-risk HPV infection.4.At the same time,Baofukang suppository can reduce the vaginal bleeding caused by mechanical stimulation with vaginal irrigation.This experiment is verified that Baofukang suppository and vaginal irrigation all have good results for patients in cervical cancer after radiation therapy suffering from vaginal adhesion.Beyond that,Baofukang suppository also has the effect of improving the cleanliness of the vagina and the anti-HPV virus.It can find a new way to prevent vaginal adhesion for patients in cervical cancer after radiation therapy,and can reduce the risk for patients in cervical cancer after radiation therapy suffering from vaginal adhesion,avoiding the pain caused by routine vaginal irrigation and the unnecessary consequences of failing to comply with the doctor’s orders,to improve the patients’ qualities of life. |
