Functional dyspepsia(FD)is a type of functional gastrointestinal disease with a high incidence and repeated the course of disease.Its pathogenesis is still elusive,the current brain-intestinal-Microbial axis theory is widely accepted as a large number of domestic and study found that FD patients with anxiety and depression are more common,intestinal flora involved in anxiety and depression,other studies found that inflammatory cytokine IL-1? involved in gastrointestinal motility and visceral sensory regulation,and is associated with the pathogenesis of depression.Purpose: To investigate the relationship between IL-1? and anxiety and depression and its possible role in the pathogenesis of FD by studying the changes of serum IL-1? levels.By studying the changes of intestinal flora in the faeces,we elucidated the possible role of intestinal flora in the pathogenesis of FD and further confirmed the theory of brain-intestinal-m icroecological axis.Methds: Screening FD patients and healthy people meet the Roman IV diagnostic criteria into the experimental group and normal group,according to the SAS and SDS score,divided into FD with anxiety and / or depression(Group B),FD was not associated with anxiety and without depression(Group M),normal(Group N),each group selected 20 cases.All people were fasting 10 ~ 12 h,in the morning to take stool specimens and venous blood samples.After pooling,the ELASA method was utilized to detect IL-1? levels.Six stool samples were randomly selected was based on environment from among each group,and the structure of intestinal flora was analyzed by high-throughput sequencing.Effect: 1.The level of serum IL-1? in Group M was significantly higher than that in Group N,and the level of serum IL-1? in Group B was significantl y higher than that in Group M(P <0.05).2.Illumina Genome Analyzer IIx 2.1 A totals of 1363399 high-quality sequences were obtained,including 4309 in N group,422973 in Group M and 509481 in Group B.The OTU,Good's coverage index and Chao1 index had no difference among the three groups(P> 0.05).Shannon index Group B > Group M,Group B > Group N(P < 0.05).2.2 The curve of species accumulation rose slowly,indicating that the sample was sufficient to estimate the richness of the community.2.3 The level of phylum,Bacteroidetes abundance Group M > Group B,Group N > Group B(P <0.05);Firmicutes abundance Group N > Group B(P <0.05).At the phylum class and order level,the dominant group of three groups are basically the same.At the family level,the three groups of dominant bacteria partially overlap(Bacteroidaceae and Ruminococca e),abundance differences(P <0.05).At the genus level,the dominant bacterias composition in the three groups was quite different.The abunda nce of Bacteroides was Group M> Group N,Group M > Group B(P < 0.05),and the abundance of Feacalibacterium was Group N > Group B(P < 0.05).Conclusion: 1.The level of IL-1? in FD patients is higher than that of normal people,and the inflammatory pathway may be involved in the pathogenesis of FD.2.The level of IL-1? in FD patients with anxiety and depression is higher than that of FD patients without anxiety and depression and normal people,and the anxiety and depression may aggravate the symptoms of FD through inflammatory pathways.3.Compared with the normal population,the structure and abundance of intestinal microflora in FD patients are different,the abundance of Bacteroidetes is increased,and the changes of intestinal microflora may be involved in the pathogenesis of FD.4.Compared with the normal population and FD patients without anxiety and depression,the structure and abundance of intestinal microflora in FD patients with anxiety and depression are different,and the abundance of the genus Clostridium difficulty is decreased,indicating that anxiety and depression may be The symptoms of FD are exacerbated by changes in intestinal microflora.
