| Professor Wang Chenglong has engaging in traditional Chinese medicine for nearly thirty years,he has accumulated rich clinical experience in the treatment of coronary heart disease with depression.Firstly,we did the xinyue jieyu tang’s theoretical research of professor Wang Chenglong’s experience prescription in treating coronary heart disease(CHD)combined with depression,to extract academic theories and clinical experience,which have the guiding significance for the clinical work;Then 35 patients suffering from CHD and depression were selected,who were treated with xinyuejieyu tang and compared with self-control study,to observe the clinical efficacy.This research is divided into two parts.The part Ⅰ:The theoretical study of xinyue jieyu tang in the treatment of coronary heart disease combined with depressionThere are no disease names of CHD and depression in Chinese medicine.According to the disease character,professor Wang believes that it can be classed as chest stuffiness and the narrow sense of depressed syndrome in Chinese medicine.①In terms of pathogenesis,professor Wang thinks that qi stagnation and blood stasis are the basic pathogeneses of the disease.However,in the process of disease development and change,others pathogeneses may occur in early stage,such as stagnated qi transforming into fire,blood stasis water stop and phlegm-heat;The deficiency of qi and blood is the main clinical manifestation in the late stage.②In terms of treatments,according to the characteristics of heart and liver in intercommunicating meridians,mutually reinforcing physiological function and interactional pathology,professor Wang advocates that harmonizing liver is the key to treating the disease.It is the main treatment method to soothe the liver and regulate qi.The method of promoting blood circulation and removing blood stasis is secondary.③In terms of prescriptions,based on the clinical experience,he invented xinyuejieyu tang.This prescription is based on the yueju pill,which focuses on the application of herbs with soothing liver and regulating qi.④On the choice of medicinal herbs for activating blood,he suggests to select drugs with functions of promoting blood circulation and regulating qi.⑤In terms of complications,insomnia and constipation cannot be neglected.It is recommended to apply the methods of nourishing blood and tranquilization,coordinating heart-kidney and relaxing bowels,rather than purgation method.⑥Because patients are mostly the middle-aged and elderly,whose physiological functions are declining gradually,professor Wang advocates that medicinal herbs of supplement the kidney and gas should be used,in order to strengthen the vital qi.Besides,in order to achieve better clinical effect,he advises that combining Chinese medicine pharmacology research choose targeted drugs on the basis of the dialectical treatment.The part Ⅱ:The clinical research about the effects of xinyue jieyu tang on the treatment of CHD with depression(qi stagnation and blood stasis type).Objective:To observe clinical effects of xinyuejieyu tang on treatment of CHD combined with depression(qi stagnation and blood stasis type),providing clinical basis for prevention and treatment of CHD with depression of Chinese medicine.Methods:35 cases were selected,which meet the inclusion criteria of CHD and depression.Patients were treated with xinyuejieyu tang on the basis of the general western medicine treatment of CHD.The clinical course was 4 weeks.By comparing with HAMD scores,Seattle Angina Questionnaire(SAQ)scores and traditional Chinese medicine(TCM)syndrome scores before and after therapy,we can evaluate the clinical efficacy of xiyuejieyu tang.Results:HAMD scores(except for weight item),SAQ scores and TCM syndrome scores of the patients after four weeks treatment,compared with before treatment,have obvious difference(P<0.01).Obvious adverse reactions were not observed.Conclusion:xinyuejieyu tang can obviously reduce depressive symptoms,improve the angina symptoms and the capacity of the patients’ dailylife. |
