[Objective]To explore the difference of serum biochemical indexes related to premature coronary heart disease and non-premature coronary heart disease,so as to provide an effective reference for the establishment of the risk assessment mechanism of coronary heart disease.[Methods]A total of 1911 coronary heart disease(CHD)cases in our hospital Cardiology department was selected as CHD group,including 1288 males(age 61.95± 11.13)and 623 females(age 65.09± 9.03);and selecting 10839 cases as control group from the medical examination center,including 7049 males(age 45.60±12.07)and 3790 females(age 45.66± 14.39).A total of 13 serum biochemical indexes were detected:total bilirubin(TBIL),direct bilirubin(DBIL),indirect bilirubin(IBIL),?-glutamyltransferase(GGT),uric acid(UA),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),Apolipoprotein A1(APO A1),Apolipoprotein B100(APOB100),Lipoprotein a(LP(a)),Homocysteine(HCY).1.The normal distribution of all variables is tested,and the logarithmic transformation is carried out according to the results that do not conform to the normal distribution.2.All the biochemical indexes of were analyzed of changing trend with age after stratificated by gender in both control group and CHD group.3.According to the definition of the premature CHD by NECP-ATP III(premature CHD refers to CHD occurs less than 55 year old in male,and under 65 years in female),all the research objects both in CHD group and control group were divided into under 55 years group and above 55 years old group in male,and in female it has been devided into less than 65 year old group and over 65 years old group.According to the groups that has been stratificated by gender and age,the difference of variables between CHD group and control group was compared.Meanwhile,Pearson correlation analysis was carried out for all variables of male and female.Serum and biochemical indexes about premature CHD and non-premature CHD both in male and female were screened out.4.The serum biochemical indexes selected above were used as independent variables to carry out to binary Logistic regression analysis in each group.[Results]1.UA,TC,HDL-C,LDL-C,APOA1 and APOB100 are basically in the normal distribution and are analyzed directly using the original data.TBIL,DBIL,IBIL,GGT,TG,HDL-C,APOB100,LP(a)and HCY does not conform to normal distribution and carries out analysis after logarithmic transformation.2.There were gender and age differences in all serum biochemical indexes.3.Combine the comparison results and pearson correlation analysis,InTBIL,InGGT,UA,lnTG,InHDL-C,InLP(a)and InHCY will be selected into the subsequent analysis of premature CHD in male;and in non premature CHD group of male,lnTBIL,UA,TC,lnTG and InHDL-C are selected;InTBIL,lnGGT,UA,TC,lnTG lnHDL.C,LDL-C,lnLP(a)and InHCY are included in subsequent analysis of female premature CHD group;while in female non premature CHD group,InTBIL,lnGGT,TC,lnHDL-C and InHCY are selected.4.The serum biochemical indexes of premature CHD in male and female are to the same,namely InTBIL,InGGT,InTG,InHDL-C,InLP(a)and InHCY;InTBIL,UA,TC,lnTG and InHDL-C are associated with non premature CHD in male;while in female non premature CHD,the related serum biochemical indexes are InTBIL,InGGT,InHDL-C and lnHCY.[Conclusions]1.Gender and age differences are existed in the risk factors of biochemical markers that CHD related.2.The risk factors between premature CHD and non-premature CHD are different.3.It is necessary to distinguish between gender and age when establishing the CHD risk assessment mechanism.
