Objective:To investigate the effect of on the renal function after blocking cardiopulmonary bypass under deep hypothermia(20~25 ?)compared to moderate hypothermia(26~31 ?)during aortic dissection operation.Method:We performed a retrospective analysis including 140 aortic dissection patients who received Stanford A aortic arch replacement from January 2013 to December 2017 in the second hospital of Jilin University.Patients were divided into deep hypothermia group(N=70)for 20?~25? and moderate hypothermia group(N=70)for 26?~31 ? according to their rectal temperature during extracorporeal circulation.Recorded and analyzed the general condition before operation, operative time, cardiopulmonary bypass time, cooling time, rewarming time, interruption time, total hospitalization days, postoperative serum creatinine, blood urea nitrogen(BUN)and CRRT data of two groups of patients. The conclusion is obtained by comparing and analyzing the data.Result:General condition of patients before operation included gender, age, cardiac function, complication(hypertension, diabetes)and preoperative serum creatinine and blood urea nitrogen value. There was no statistical difference in the operation time, extracorporeal circulation time, ascending aorta blocking time and stopping cycle time in the two groups.Cooling time and rewarming time in moderate hypothermia group were shorter than deep hypothermia group. There was no significant difference in postoperative urine volume,CRRT ratio, mortality rate and total hospitalization time between two groups. There was a significant difference in serum creatinine and BUN value between the two groups. The values of deep hypothermia group were higher than those of moderate hypothermia group. In the two groups, the value of serum creatinine and BUN increased within one week after operation, and the blood creatinine value in the deep hypothermia group was higher than that in the moderate hypothermia group. The blood creatinine and BUN value of the two groups decreased from 8th to 10 th days after operation. There was no significant difference of preoperative creatinine and BUN,creatinine value and BUN value of 10 days after surgery between the two groups. Conclusion:Moderate hypothermia and deep hypothermia can lead to renal function change after operation for type A aortic dissection. However, moderate hypothermia had fewer effect on compared with deep hypothermia.
