AimAnalyze the early diagnostic efficiency of each index in the blood routine test for gastric cancer,and compare the effect of Helicobacter pylori infection on blood routine indexes.MethodsA total of 223 patients and hospitalized patients with gastroscopy were selected from September 2016 to September 2017,and were divided into four groups according to gastroscopy and pathological results,58 patients in chronic superficial gastritis(CSG)group,59 patients in chronic atrophic gastritis(CAG)group,52 patients in high-grade intraepithelial neoplasia(HIN)group,57 patients in gastric cancer(GC)group.Patients with cardiovascular disease,hematological disease,immune system disease,infection and other cancer were excluded from the study.Comparison the differences of red blood cell distribution width(RDW),platelet count(PLT),mean platelet volume(MPV),platelet distribution width(PDW)and white blood cell count(WBC),lymphocyte count(LYMP),neutrophil counts(NR),neutrophil count to lymphocyte ratio(NLR),neutrophil count to lymphocyte ratio(PLR)in different groups,and compare the various index sensitivity,specificity of diagnosis of gastric cancer;At the same time,according to the result of Helicobacter pylori(Hp)antibody,each component was divided into two subgroups:Hp antibody positive and Hp antibody negative.Comparing the infection rate and blood routine index in each subgroup.Result1.MPV?LYMP decreased in CAG group?HIN group?GC group.while RDW?NLR?PLR gradually increased.2.RDW?MPV?LYMP?NLR?PLR in CSG group and GC group,the differences were statistically significant(P<0.05).The difference of PLT?PDW?WBC?NR in each groups were not statistically significant.3.In detecting GC,the sensitivity of RDW?MPV?LYMP?NLR?PLR?NR were 82.5%?80.7%?84.2%?84.2%?71.9%?45.6%,respectively;specificity were 72.4%?55.2%?82.8%?91.0%?89.7%?89.7%,respectively.4.RDW,MPV,LYMP,NLR and PLR are significant for identifying GC and HIN(P<0.05),Area under ROC curve respectively were 0.745?0.635?0.716?0.730?0.695;RDW,MPV,LYMP,NLR and PLR are significant for identifying GC and CAG(P<0.05),Area under ROC curve respectively were 0.807?0.663?0.732?0.734?0.763.5.The positive rates of Hp in CSG group,CAG group,HIN group,and GC group were 63.7%,62.5%,63.5%,and 66.7%respectively.Among them,the positive rate of Hp in GC group was the highest.There was no significant difference in the positive rate of Hp between the groups(P>0.05).6.In CSG group,the PLT?WBC?NR in Hp positive patients were highter than those in Hp negative patients(P<0.05).There was no significant difference between RDW,MPV,PDW,LYMP and NLR between the two subgroups.There were no significant differences between Hp positive and Hp negative patients of CAG,HIN and GC group in blood routine indexes(P>0.05).ConclusionRDW?PLT?MPV?LYMP?NLR?PLR can be used in the diagnosis of early gastric cancer;In the superficial inflammation of the gastric mucosa,PLT,WBC,NC levels are associated with Hp infection.When the gastric mucosa is atrophic,intestinal metastatic,and cancerous,the changes of blood routine markers and Hp infection is not relation.
