| Objective:Object:To explore the value of high frequency ultrasonography in the diagnosis of ankle ligament injury.Methods:The ultrasonography with high-frequency liner array probe was performed to examine the patients with ankle sprain,espacially to the collateral ligament injury.The ultrasonography findings were compared with the surgical results in the patients who were surgical patients.Results:In 59 cases who suspected ankle joint injuries,42 cases were positive ankle ligament injuries and 17 cases were negative.There are 8 patients with type I injury,and 14 cases with type II injury,and 20 cases with type Ⅲ injury.The surgical operation,arthroscopic surgery and clinical follow-up confirmed what the detection of high frequency ultrasonography had found:40 cases of ligaments injury were true positive,and 2 cases were false positive,13 cases were true negative,and 4 cases were false negative,with the diagnosis sensitivity being 90.90%,the specificity being 86.67%,the accuracy rate of 89.83%,the positive predictive value being 95.2%,and the negative predictive value being 76.5%,P= 0.687(p>0.05).In the 22 cases with 46 ligaments were reconstructed,21 ligaments were type Ⅱ ankle ligament injury,the coincidence rate of high frequency ultrasonography was 85.7%(18 ligaments),P=0.581 and the coincidence rate of MRI was 90.4%(19 ligaments),P=0.754;25 ligaments were type Ⅲankle ligament injury,the coincidence rate of high frequency ultrasonography was 84.0%(21 ligaments),P=0.219,and the coincidence rate of MRI was 92,0%(23 ligaments),P=0.625.P value was greater than 0.05,and the difference was not statistically significant.Conclusion:High frequency ultrasonography has a high clinical value in the diagnosis of collateral ligament damage in the ankle joint,which is helpful for accurate classification and provides an objective basis for the selection of clinical treatment modality and prognosis. |
