Objective:kidney transplant is the most effective way to improve the quality of life in patients with end-stage renal disease,the quality of the donor kidney is the decisive factor of the success of kidney transplant,so correct evaluation for kidney quality is critical.In this study,a comparative study was conducted on the preoperative evaluation of DBD donor renal ultrasound and the postoperative recovery of the trans-planted kidney,and the value of ultrasound in the evaluation of renal transplantation in DBD kidney transplantation was discussed.Methods:Collected from January 2014 to October 2017,our hospital clinical data donor kidney and ultrasound evaluation data,including renal length to diameter,wide diameter,material thickness,thickness of the cortex,the collection system and renal aorta and segmental arteries,leaf between the peak systolic velocity(PSV),blood flow resistance index(RI),donor ultrasonic data can be divided into normal group(group A)and abnormal group(group B),and corresponding to the recipients transplanted renal ultrasound postoperative data and the evaluation of the clinical data of postoperative recovery.The postoperative recovery was divided into two groups:recover well and recovery exception,through to the donor and receptor one-to-one retrospective analysis and comparative study,to explore the value of ultrasound in assessment of DBD donor kidney.Results:There were 83 cases in the donor A group,about 66.9 percent.Long diameter of Its morphological data was 10.48±1.27cm,wide diameter was 4.75±0.51cm,parenchyma thickness was 1.76±0.34cm,cortex thickness was 0.94±0.23cm;There were 41 cases in the donor A group,about 33.1 percent.Long diameter of Its morphological data was 10.37±2.29cm,wide diameter was 4.87±1.08cm,parenchyma thickness was 1.79±0.47cm,cortex thickness was 0.93±0.31cm.According to the statistical analysis,P>0.05,there was no statistical difference between the two groups.The recovery in the donor A group was good 69 cases,accounting for 83.1%,and the recovery was abnormal in 14 cases,accounting for about 16.9%.In group B,23 cases were recovered,about 56.1%,and 18 cases were recovered in group B,about 43.9%.Using Chi-square test A and B two groups of donor kidney transplantation recovery statistical analysis,the result shows:P =0.001.The recovery rate was about 77.1%with 5 and below anomaly indicators in all evaluation indexes,the recovery rate was about 16.7%with 6 or 7 anomaly indicators in all evaluation indexes.In this study,there were no donor kidneys with 8 or more abnormalities.In group A and B to single factor analysis of variance of artery PSV at all levels,renal aorta 75.71 ±21.24 cm/s,84.95±29.75cm/s,P = 0.049,segmental arteries 47.71±15.34 cm/s,59.28±26.68 cm/s,P = 0.003,interlobar arteries 31.04±10.93cm/s,37.33±17.57cm/s,P = 0.016,all P values<0.05,the difference is statistically significant.Therefore,it can be considered that the PSV of the renal arteries of the donor kidney can be used to evaluate the quality of the donor.Conclusion:1.Ultrasound examination can be convenient,quick and noninvasive to evaluate renal transplant donor kidneys,the donor kidney ultrasound evaluation of quality also have closer relations with the recovery of graft.2.With 5 and below anomaly indicators in all evaluation indexes its recovery after transplantation is better,can choose as a donor,while with 6 or 7 anomaly indicators in all evaluation indexes kidney transplantation is poorer,used as the donor is not recommended.3.The PSV of the renal arteries of the donor kidney was used to evaluate the quality of the donor.It can guide the clinical better selection of donor and precautionary treatment of postoperative transplant patients,prevention and treatment of complications,and improve the success rate of renal transplantation.
