ObjectivesTo explore the prevention and treatment of acute gouty arthritis with the combination of total glucosides of paeony and total saponins of Panax notoginseng,so as to provide reference for clinically and effectively creating new prescriptions.Methods96 male SD rats were randomly divided into normal group,model group,colchicine group,total glucosides of paeony group,total saponins group of Panax notoginseng and total saponins of Panax notoginseng group.16 in each group.Each group was pre-administrated intragastrically for 5 days,and on the 6th day after administration,sodium iodate crystals were injected into the rat's ankle joint,except for the normal group.The paw volume was measured and recorded at different time points after modeling.At the 24 th hour after modeling,8 rats were randomly selected from each group to take blood after abdominal anesthesia,and ankle joint specimens were taken.The remaining rats were continuously intragastrically administered.At the 144 th hour after modeling,the remaining rats were euthanized after taking blood to take an ankle joint specimen.Observe the changes of the related indicators of the rats before and after the administration of the drug and the model.Observe the pathological changes of the sacroiliac joint of rats by HE staining.Detect the content of IL-1?,IL-18 and TNF-? in the serum by ELISA.Detect the joint by immunohistochemistry.Expression of NLRP3,Caspase-1,IL-1?,IL-18,TNF-?,and COX-2 in synovial tissue.ResultsThe combination of total glucosides of paeony combined with Panax notoginseng saponins exerts good anti-inflammatory and anti-swelling effects within 24 hours after modeling,and can reduce the rate of swelling and increase of ankle joints in rats after modeling.Compared with the model group,the swelling rate of sacroiliac joints in the combination of total glucosides of total saponins of P.striata 36 hours after modeling was relieved,and was lower than that of Panax notoginseng saponins group at 36,72,and 96 hours after modeling(P<0.05).There was no significant difference in the swelling rate between the total glucosides of total glucosides of P.sinensis and the total saponins of Panax notoginseng group after 144 hours.The contents of IL-1?,IL-18 and TNF-? in the colchicine group,total glucosides of paeony group,total saponins of Panax notoginseng and total saponins of total paeony combined with model group were significantly higher than those of the model group.The difference was statistically significant(P<0.01).Observation of pathological changes in the model group showed neutrophil and other inflammatory cell infiltration in the joint group,and synovial tissue edema.The pathological changes of the TGP-combined PNS group were improved compared with the model group,and there was no morphological abnormality in the normal control group.The expression of NLRP3,Caspase-1,IL-1?,IL-18,TNF-? and COX-2 in the sacroiliac joint of rats in each administration group was significantly lower than that in the model.Conclusions1 The combination of total glucosides of paeony combined with Panax notoginseng saponins can improve the joint pathological changes of acute gouty arthritis in rats and inhibit the swelling of the joints.The compatibility of the two drugs has a synergistic effect and the effect is better than the two alone.2The possible mechanisms for the combination of total glucosides of paeony and total panax notoginsenosides in treating AGA in rats are: inhibition of the expression of NLRP3,Caspase-1,IL-1?,IL-18,TNF-? and COX-2 in joint tissues,and downregulation of serum IL-Expression of inflammatory cytokines such as 1?,IL-18,and TNF-?.
