Congenital varus,valgus,acute trauma,rheumatoid arthritis,knee joint degeneration can cause injury and dysfunction of the knee joint movement structure in different degree,reduce the quality of life of patients,while in the normal population,the knee injury also exists,especially in the risk of knee joint activity and heavy manual workers and the athlete high damage,with 45-60 years old middle-aged people.The currently used clinical knee arthroplasty for congenital knee valgus,acute trauma,rheumatoid,osteoarthritis,knee joint degeneration and excessive movement caused by knee joint injury treatment,knee joint angle osteotomy during operation and prosthesis design an important step to ensure that the success of total knee replacement arthroplasty.Therefore,in order to improve the curative effect of the operation and the expected quality of life in patients with total knee replacement surgery,preoperative need conduction on patients with knee joint shape and size and power line For imaging evaluation,in order to achieve the desired effect of surgery,digital image database to establish normal population parameters of knee joint is very important,China's population distribution latitude span,regional and ethnic distribution is obvious,on a regional distribution of normal knee joint parameter imaging,this study by X-ray measurement Shaanxi normal middle-aged double knee joint parameters,use of local normal middle-aged and normal knee joint shape parameters and scholars at home and abroad are compared,the total knee arthroplasty osteotomy angle and knee prosthesis design provides a certain reference value.The imaging data of normal middle-aged people aged 45 to 60 years were performed in our hospital from October 2014 to October 2017.According to the inclusion and exclusion criteria of this study,(1)there were no obvious symptoms such as knee pain,swelling and strangulation;(2)No previous trauma and surgical history.(3)X-ray examination of knee joint showed no obvious deformities,obvious bone degeneration and hyperplasia,and no extroversion deformity.A total of 91 cases with 182 genuses were obtained.Among them,50 cases were female,and 82 cases were male.The average age was 50.29±5.51 years old.The data were measured and studied.The length indexes of the X-ray measurement parameters of knee joint were as follows: transverse diameter of femoral condyle,transverse diameter of tibial plateau,vertical distance from vertical axis of proximal tibia to the lowest point of tibial plateau,transverse diameter of patella,vertical diameter of patella,upper and lower diameter of patella.Anterior and posterior diameters of femoral condyle,anteroposterior diameters of tibial plateau,thickness of anterior femoral condyle and patellar thickness;Measurement angle index: femoral tibial valgus angle(the angle between distal femur and proximal tibia central axis,distal femoral eversion angle(distal femur axis perpendicular to femoral internal and external condyle tangent angle)PT angle(vertical line of proximal tibia axis line)The angle between the vertical line of the midaxis of the proximal tibia and thetangent of the internal and external condyle of the femur,the angle of the posterior inclination of the tibial plateau(X ray lateral radiography of knee joint,the angle of the angle between the lowest tangent line of the internal and external sides of the tibial plateau)The angle between the vertical line of the central axis of the proximal tibia and the tangent line of the medial anterior and posterior edge of the tibial plateau was introduced into the SPSS22.0 software for statistical analysis,and the data obtained were all consistent with the normal distribution,so the gender and the left and right sides were taken as variables.Using the single factor variance analysis method,when the test standard is P <0.05,the measured data are different and have statistical significance.In this study,12 items of X ray measurement parameters of knee joint of normal middle-aged in Shaanxi area were set,including 7 items of length parameter and 5 items of angle parameter.The sample size of 91 cases of knee joint was 182 knees,and the statistical data was 2184 items.The results show that the statistical results of knee joint parameters in this area are expressed as mean ±standard deviation.the lateral femoral condyle was 8.08 ±0.39(cm)in the left side,8.13 ± 0.39(cm)on the right of male;7.08 ± 0.28(cm)on the left side and 7.08 ± 0.55(cm)on the right side of female.The lateral diameter of the tibial plateau was 8.41 ± 0.40(cm)on the left side,8.45 ± 0.45(cm)on the right side of male;7.38± 0.30(cm)on the left side and 7.42 ± 0.32(cm)on the right side of the female;The DPF was 0.62 ± 0.31(cm)on the left side,the right was 0.37 ± 0.36(cm)of male,the left side of the female was 0.59 ± 0.30(cm),the right was 0.62 ± 0.34(cm);The lateral diameter of the male was 5.43 ± 0.48(cm)on the left,5.36 ± 0.44(cm)on the right,4.79 ± 0.34(cm)on the left side and 4.79 ± 0.33(cm)on the right side of female;The anterior and posterior tibial plateau was 5.58 ± 0.39(cm)on the left side,5.58 ± 0.35(cm)on the right of male;4.84 ±0.35(cm)on the left side and 4.82 ± 0.34(cm)on the right side of female;The anterior femoral condyle was 0.72 ± 0.22(cm)on the left side,0.69 ± 0.22(cm)on the right side of male,0.69 ± 0.16(cm)on the left side and 0.68 ± 0.18(cm)on the right of female.The Patellar thickness was 2.29 ± 0.16(cm)on the left side and 2.29 ± 0.15(cm)on the right side of male,2.08 ± 0.14(cm)on the left side and 2.06 ± 0.12(cm)on the right side of female.The femoral tibial valgus angle was 5.95 ±2.39(°)in left,4.01 ±2.36(°)in right of male;6.81 ±2.82(°)in left,4.75 ±2.33(°)in right of female.The distal femoral valgus angle was7.46 ±0.98(°)in left,6.27 ±1.24(°)in right of male,7.78 ±1.46(°)in left,6.40 ±1.18(°)in right of female,The PT was 3.43 ±1.15(°)on the left and 5.57 ±1.29 on the right of male,3.46 ±1.17(°)on the left and 3.38 ±1.19(°)on the right of female.The FT was 4.07 ±1.39(°)on the left side and the right was 4.06 ±1.27(°)of male,the female's left was 3.91 ±1.37(°),the right was 4.02 ±1.28(°);the male tibial plateau was 9.71 ±2.16(°)on the left side,the right was 9.38 ±2.14(°)of male,the female's left was 9.50 ±2.23(°),the right was 9.31±2.59(°).The statistical data of this study showed that the data obtained from each measurement index had a good concentration.Through statistical analysis,the resultsshowed that: male and female left side,male and female left right side were compared with each other.The transverse diameter of femoral condyle,tibial plateau,anteroposterior diameter of medial and medial femoral condyle,anteroposterior diameter of tibial plateau,transverse diameter of patella,superior and inferior diameter of patella,patellar thickness were all significantly different(P < 0.05).There was no significant difference in DPF and angle index of femoral anterior condyle thickness(P > 0.05).Using the left and right side as variables,the data were analyzed.The results showed that the femoral tibial valgus angle was significantly different between male and right side,female left side and right side(P <0.05),and PT angle was FT angle.There was no significant difference in posterior inclination angle and length of tibial plateau(lateral diameter of femoral condyle,transverse diameter of tibial plateau,anteroposterior diameter of medial femoral condyle,anteroposterior diameter of tibial plateau,transverse diameter of patella,superior and inferior diameter of patella,thickness of patella)(P < 0.05).The results of knee joint data measurement in this study are similar to those of other regions in China,and the data concentration is good and the dispersion degree is low,which can reflect the range of knee joint data of normal middle-aged people in Shaanxi area.It provides a theoretical reference for preoperative evaluation of total knee arthroplasty and design parameters of knee prosthesis,and fills the gap in the study of knee joint data in Shaanxi area.It provides important theoretical basis and practical value for the operation of total knee arthroplasty in this area.
