Objective:To investigate the application value of vascular caliber in identifying the benign and malignant thyroid nodules by superb microvascular imaging(SMI)technology.And investigate the clinical value of SMI combining with fine needle aspiration(FNA)in diagnosis of thyroid cancer.Methods:Totally 105 patients with 111 thyroid nodules were randomly selected,which were divided into two groups respectively according to the size of the nodules,and the grouped criterion was 2 cm.SMI software was used to measure the blood vessel diameters in the thyroid nodules,then respectively obtained the cut-off value in diagnosing thyroid cancer.And totally 52 patients with 54 thyroid nodules were selected.Firstly,the SMI software was enabled to determine nodule properties based on the cut-off values obtained from the pre-experiment.Then start FNA,according to the Bethesda system for reporting thyroid cytology for classification,pathology as the gold standard to contrast SMI and FNA both alone and in combination to assess the efficacy of thyroid cancer.Results:In the group which the value of vascular caliber was less than or equal to 2 cm,the average blood vessel diameters of benign group and maligant group were separately(0.90±0.13)mm,(0.61±0.17)mm,so there were significant differences between two groups(P=0.000),the diagnostic threshold was 0.75 mm,the sensitivity was 77%,the specificity was 85%.In the group which the value of vascular caliber was larger than 2cm the corresponding values were separately(1.12±0.19)mm,(0.76±0.20)mm,so there were also significant differences between two groups(P=0.000),the diagnostic threshold was 0.95 mm,the sensitivity was 76%,the specificity was 83%.According to the above threshold,the sensitivity was 80.0%,the specificity was 78.6%and the accuracy rate was79.6%of SMI in diagnosis of the other 54 lesions.The sensitivity was 85.0%,the specificity was 85.7%and the accuracy rate was 85.2%of FNA in diagnosis of the nodules.The sensitivity was 95.0%,the specificity was 92.9%and the accuracy rate was94.4%of the both in diagnosis of thyroid cancer.The diagnostic accuracy of FNA was slightly higher than that of SMI(?~2=4.91,P=0.03),the diagnostic accuracy of both was higher than that of SMI and FNA alone(?~2=13.25,P=0.00;?~2=33.65,P=0.00).Conclusion:The SMI technique can effectively measure the diameter of blood vessels and is of great value in diagnosis of thyroid cancer.Combined with FNA,it can improve the diagnostic efficiency of thyroid cancer.
