The Analysis Of 533 Cases Of Postoperative Thyroid Nodule Over 40 Years Old

Objective: To analyze lesion characteristics in thyroid nodule pathologyical feature of above 40-year old, and to evaluate the value in diagnosis thyroid nodule among the ultrasound assessment, fine-needle aspiration biopsy.Methods: Collected the clinical data about above 40-year old thyroid surgical patients with hospitalized with thyroid nodules surgical treatment in Affiliated Hospital of Guiyang Medical College from Feb,2011 to Dec, 2013.Sorted out the patients with Ages, sex, pathology ic type, the benign/ malignant proportion of thyroid nodule, to analyst the relationship between thyroid stimulating hormone(TSH) before surgery with the benign/malignant thyroid nodule; and based the result of pathology after operation as a standard to evaluate the accuracy by the method of preoperative ultrasound, fine needle aspiration biopsy diagnosis; Using the benign / malignant thyroid nodule as the dependent variables to predicting the risk factors in malignant thyroid nodules via using the multivariable logistic regression analysis method.Results:(1)Thyroid nodule is very common in people with aged from 40 to 59, and the male-female ratio is 1:5.92.(2) Malignant in thyroid nodule in male-female ration is 23.4%: 13.6%.(3) In pathologic- al types of thyroid nodule, the rate of nodular goiter and adenoma take up 68.3%; the papillary thyroid carcinoma takes up to 13.5%; and the follicular carcinoma,medullary carcinoma of the thyroid and the others only take up 1.6%.(4) Rate of malignant with single, solid, great nodule is higher than the multiple, mixed, but it is lowering in cystic;(5). The coincidence rate between the Postoperative pathological results with the result of preoperative ultrasound & fine-needle aspiration biopsy respective are 82.8% and 94.9%;(6) With the increase of the TSH, the malignant nodule also rises.Conclusion: The rate of malignant thyroid nodule in male is higher than female with 40-year old, and it is a very important diagnosis value to evaluate the benign / malignant thyroid nodule with the method of preoperative ultrasound, fine-needle aspiration biopsy.
