Serum Fetuin-B Levels And Influencing Factors In Newly Diagnosed Type 2 Diabetes Mellitus

Objective: Fetuin-B plays an important role in the development of metabolic diseases.In this study,serum Fetuin-B levels in patients newly diagnosed type 2 diabetes mellitus(T2DM)were determined and analyzed its relationship with body mass index(BMI),waistline(WC),fasting blood glucose(FBG),blood lipids,uric acid(UA),homeostasis model assessment of insulin resistance(HOMA-IR),superoxide dismutase(SOD),malondialdehyde(MDA),glutathione glycine peroxidase(GSH-PX)、Non-alcoholic fatty liver disease(NAFLD)and atherosclerosis(AS)and Provides the basis for prevention of type 2 diabetes and its complications.Methods: Select 203 newly diagnosed patients with type 2 diabetes in the Department of Endocrinology of Hebei Provincial People’s Hospital from January 2017 to December 2017;The healthy control group(CON)consisted of 209 healthy people of the same period.T2 DM were divided into atherosclerosis subgroup(A group)and non-atherosclerotic subgroup(B group)according to the carotid atherosclerosis.And T2 DM were divided into NAFLD(C group)and non-NAFLD subgroup(D group)according to the carotid NAFLD.Fasting blood glucose(FBG)and blood lipid were measured by automatic biochemical analyzer.The level of serum insulin was detected by electrochemiluminescence assay.The glycosylated hemoglobin(HBA1c)was determined by high pressure liquid chromatography.Fetuin-B was detected by enzyme-linked immunsorbent assay(ELISA)kit.WST-1 method was used to measure serum total superoxide dismutase(T-SOD),serum MDA level was measured by TBA method,and GSH-PX activity was measured by colorimetric method.Insulin sensitivity was assessed using homeostasis model assessment of insulin resistance(HOMR-IR).Data were analyzed using SPSS 21.0 software.Results:1.The association of the baseline clinical characteristics between CON and T2DMCompared with the CON group,BMI、WC were increased in T2 DM group(P <0.001).And the levels of TC、TG、LDL-C、APOB were higner than that in CON group(P<0.05).The levels of HDL-C、APOA were lower than in CON group(P<0.05).Compared with the CON group,FBG、HOMA-IR were increased in T2 DM group(P<0.001).The level of GSH-PX in T2 DM group was significantly higher than that in CON group(P <0.001),while SOD was lower than that in CON group(P <0.001).The VAI and LAP in T2 DM group were significantly higher than those in CON group(P <0.001,P <0.001).Serum uric acid(UA)in T2 DM group was higher than that in CON group(P <0.05).Fetuin-B in T2 DM group was significantly higher than in CON group(P <0.001).2.Comparison of each index between two subgroups(A、B group)in T2 DM group.There was no significant difference in BMI,WC,VAI,LAP,FINS,GSH-PX,UA,serum lipids,bilirubin and among groups A and B(P> 0.05).The FBG、HOMA-IR、SOD,in group A were lower than those in group B,MDA was higher than those in group B and there were significant differences(P <0.05).There was no significant difference in serum Fetuin-B between groups A and B(P> 0.05).3.Comparison of each index between two subgroups(A、B group)in T2 DM groupComparing C,D subgroups in age,gender,HbA1 c,FBG,blood lipids,bilirubin,UA,GSH-PX,MDA,SOD,VAI,etc.There was no statistically significant difference(P> 0.05).C subgroup BMI,WC,FINS,HOMA-IR,LAP is higher than D subgroup,and there is a statistical difference(P <0.05).The serum Fetuin-B in subgroup C was higher than subgroup D and there was a statistical difference(P<0.001).4.Association analysis of serum Fetuin-B with general data,biochemical indicators and oxidative stress indicatorsUsing spearman correlation analysis,the results showed that serum Fetuin-B was positively correlated with BMI,WC,VAI,LAP,FBG,FINS,HOMA-IR,HbA1 c,UA,TG,APOB,GSH-PX,and serum Fetuin-B was negatively correlated with HDL-C and SOD.Using multiple linear stepwise regression analysis with Fetuin-B as the dependent variable,the results showed that glycosylated hemoglobin has the greatest effect on serum Fetuin-B,which is an important factor affecting Fetuin-B.5.Correlations between various indicators and T2DMLogistic regression analysis was used in the T2 DM group and T2 DM was used as the dependent variable to observe the impact of various indicators on T2 DM.The results showed that the selected variables were Fetuin-B,FBG,GSH-PX,and The P values were all <0.05.The OR values are 1.001,12.552,1.09,respectively.Logistic regression equation has statistical significance.The results suggest that Fetuin-B,FBG,and GSH-PX are risk factors for T2 DM.6.The relationship between serum Fetuin-B levels and the incidence of T2 DM.All subjects were divided into four groups according to the serum Fetuin-B quartile.From Q1 group to Q4 group,the incidence of T2 DM was 17.0%,30.2%,54.7%,and 92.5%,respectively.The results suggest that compared with Q1 group,the incidence of T2 DM increased in Q3 and Q4 groups,and the difference was statistically significant(P<0.001).There was no significant difference between Q2 and Q1 groups(P=0.109).Model 1 uses the Q1 group as a reference.Compared with Q3 and Q4 groups,the incidence of T2 DM increases.After Model 2 corrects GSH-PX,and HDL-C,the incidence of T2 DM in Q3 and Q4 groups is still higher than that of Q1.Group;Model 3 further adjusted the FBG,Q3,Q4 group T2 DM risk is still higher than Q1 group.7.The correlation between each index and T2 DM combined NAFLD subgroupLogistic regression analysis was applied to the T2 DM patients with NAFLD subgroup(C group).T2 DM with NAFLD was used as a dependent variable to observe the effects of various factors on T2 DM with NAFLD.The results showed that the selected variables were Fetuin-B and BMI,P values were 0.000,0.018,P values were all <0.05,and OR values were 1.001 and 1.558,respectively.Therefore,Logistic regression equation has statistical significance.The results suggest that Fetuin-B and BMI are risk factors for T2 DM with NAFLD.Conclusion:1.The level of Fetuin-B in serum of newly diagnosed type 2 diabetic patients was increased,and the level of Fetuin-B in serum of newly diagnosed type 2 diabetes patients with NAFLD was higher than that of patients without NAFLD,which was the influencing factor of type 2 diabetes.2.Glycated hemoglobin is an important influence factor of Fetuin-B.
