Objective:To summarize the experience of surgical treatment of tracheal diseases in 26 cases to reduce perioperative complications.Methods:The clinical data of 26 patients with tracheal diseases treated in our department from 2002 to 2017 were retrospectively analyzed.The patients were 37-76 years old and the average age was 41 years old.There were 17 males and 9 females.A total of 17 tracheal tumors were identified,including 14 primary malignant tracheal tumors?9 squamous cell carcinomas and 5 adenoid cystic carcinomas?,3 secondary tumours?all of which were thyroid papillary adenocarcinomas invading the trachea?,and 1 benign tumour.?for extramedullary plasmacytoma?.8 cases of non-neoplastic diseases,including traumatic tracheal laceration after scarring in 5 cases,tuberculosis to scar stenosis in 1 case,tracheal intubation rescue tracheal stenosis in 2 cases?intubation time:1 case of 18days,1 case of 15 days?.Results:No operation or perioperative death occurred in 26 patients in this group.Twenty-five cases of endotracheal resection were performed in patients with tracheal tumor or stenosis.One case of invading the trachea with cancer,and general surgery cooperation line radical surgery+tracheal window resection.All patients were cured or improved.After follow-up,the total 5-year survival rate of malignant tumors was 70.5%?12/17 cases?.Symptoms of hoarseness appeared in 2 cases of lung infection after surgery,another 4 cases had mild irritation cough.In 25 patients who underwent annular resection of the diseased segment and who had anastomosis,there was no anastomotic stricture and anastomotic dehiscence.The patient's postoperative respiratory status was good,activity tolerance was significantly greater than before,and the quality of life was significantly improved.Conclusions:Tracheal diseases usually have a greater impact on the respiratory function of patients.All tracheal stenosis and tracheal tumor surgery are performed in sub-emergency surgery[15].In particular,patients with severe airway obstruction usually require rescue intubation and emergency surgery.At present,the most effective treatment for tracheal tumors is surgery.Among them,the most important surgical method for tracheal malignancy and tracheal stenosis is anastomosis of tracheal sleeve resection end,and conservative techniques such as local trachea resection and window resection are generally applicable to benign tracheal tumors.Surgical treatment should not only guarantee the radicality of the surgery,but also consider its safety.More importantly,it can simultaneously solve the symptoms of respiratory obstruction in patients.Tracheal surgery has its own points of surgical treatment depending on its cause.
