Objective:To explore the changes of immunoinflammatory markers in syphilis patients and their clinical significance,and to provide new ideas for the differential diagnosis of rheumatic immune diseases.Method: Thirty-one patients diagnosed as syphilis in the Department of Rheumatology and Immunology,First Affiliated Hospital of China Medical University were recruited and their immunoinflammatory indexes including Erythrocyte sedimentation rate(ESR)and serum C-reactive protein(CRP).immunoglobulin(Ig),complement(C3,C4),T cell subsets,antinuclear antibody(ANA),extractable nucler antigen(ENA),anticardiolipin antibody(ACL),anti-neutrophil cytoplasmic antibody(ANCA)were collected.Other data,including age,gender,desease duration,main related symptoms and signs,including fever,rash,joint swelling,pain,mucosal ulcers,weight loss,muscle pain,as well as routine clinical indicators,including blood routine,liver and kidney function,blood glucose,blood clotting indicators were also collected.Analyzed the changes of immuno-inflammatory markers in those patients.Result: The levels of C-reactive protein and ESR were significantly increased in the patients with syphilis.The levels of Ig A(3.87±2.46 g/L)and Ig G(17.96±6.95 g/L)were higher than the upper limit of the normal range.Of the 31 patients,15 patients were tested for anticardiolipin antibodies,6 of which were positive for anticardiolipin antibodies(40.00%).A total of 28 patients were tested for ANA and ENA.Twenty patients were positive for anti-nuclear(71.42%),five patients were positive for anti-SSA antibody positive(17.86%),five patients were RO-52 antibody positive(17.86%),three cases were histone antibody(AHA)positive(10.71%),1 case was anti-SSB antibody positive(3.57%),2 cases were anti-U1 RNP antibody positive(7.14%),1 case was anti-double-stranded DNA antibody positive(3.57%),and 1 case was anti-nucleosome P antibody positive(3.57%).Twenty patients underwent ANCA testing,and 3 of them were positive for perinuclear anti-neutrophil cytoplasmic antibody(pANCA)(15.00%).Conclusion:Serum inflammatory related indicators in syphilis patients were significantly increased,accompanied by a significant increase in autoantibody positivity.These results suggest that we should pay enough attention for the differential diagnosis betweensyphilis and connective tissue disease.
