Objective: To explore the clinical features,risk factors,diagnosis,treatment and prognosis of vaginal intraepithelial neoplasia(VAIN).Methods: The clinical data and diagnosis and treatment of 27 patients with VAIN admitted to our hospital ward and outpatient colposcopy examination room from January 2016 to December 2017 were retrospectively analyzed.Results: Patient age range: 30-70 years old,mean age: 47.56 years,median age: 49 years,menopause in 14 cases;19 cases(70.37%)of CIN or cervical cancer in the past or present;27 cases were all with papillomavirus(HPV)infection,HPV16 positive accounted for 44.44%,HPV58 positive accounted for 22.22%;seven cases of liquid-based cell thin-film cytology(TCT)test results were abnormal;different levels of VAIN in menopausal status and combined cervical disease was not statistically significant Significance(P> 0.05);There was a statistically significant difference in age(P<0.05);there was no significant difference in the incidence of VAIN in different age groups(P>0.05).Treatment:(1)RF treatment of vaginal wall lesions: 14 cases in total(2 VAIN I cases,11 VAIN II cases,1 VAIN III case),1 case of VAIN III sustained HPV 16 positive,no colposcopy examination.The rest of the review results are normal.(2)Cold knife resection: A total of 9 cases(1 case of VAIN II,8 cases of VAIN III),3 cases of sustained VIP infection after VAIN III,no colposcopy examination.(3)Drug therapy: 4 cases(3 cases of VAIN I,1 case of VAIN II),1 case of VAIN I completely lost,and the remaining 3 cases had good therapeutic effect.Treatment Outcomes: 5 cases except VAIN I 1 case lost to all cure;13 cases VAIN II were all cured;Postoperative persistent HPV infection in 4 of 9 VAIN III cases,no evidence of colposcopic evaluation,but could not evaluate whether there is potential Lesions,the rest cured.Conclusion: HPV infection and complications of cervical disease are the main risk factors for VAIN,among which positive HPV types 16 and 58 are the main pathogenic types in this region.For colposcopy,attention should be paid to vaginal wall lesions during colposcopy.The cytology-colposcopy-histopathology diagnosis and treatment procedure is applicable to VAIN.Treatment should be individualized and long-term follow-up after surgery.
