Objective: the pleiotropic effect of atorvastatin calcium has the effect of anti-inflammation,lowering blood lipid,improving vascular endothelium and even reversing plaque.For the internal carotid artery or middle cerebral artery with severe stenosis or occlusion,it is difficult to obtain good curative effect by routine dose drug therapy and risk factors control.Clinical trial to explore the safety and clinical effect of high dose atorvastatin calcium in acute ischemic stroke(AIS)with intracranial artery occlusion.Methods: from September 2016 to September 2017,168 patients with intracranial or middle cerebral artery occlusion which may caused by atherosclerosis(AS)in the Department of Neurology,second Hospital of Hebei Medical University were treated with anti-platelet aggregation and improved circulation.On the basis of routine treatment of nutrition and nerve,oral administration of atorvastatin calcium(Lipitor Pfizer Pharmaceutical Co.,Ltd.)40 mg /d,long-term drug use and follow-up.Follow up for 3 months,6months,1 year,because of various reasons to take 20 mg /d to form the control group,The other was the experimental group.Through follow-up,there were110 cases of valid data.The reasons included stopping the use of drugs,switching to drugs from other manufacturers,unable to obtain real information,and so on.The incidence of endpoint events and adverse reactions in each time period were compared statistically.Result : After 3 months and 6 months of follow-up,there was no significant difference in the incidence of endpoint events between the experimental group and the control group.There was no significant difference in the incidence of adverse reactions between the experimental group and the control group.There were several patients with higher AST and ALT.but noother adverse reactions between the two groups.Intracranial multi-vessel disease is an independent factor affecting end-point events.Conclusion:atorvastatin calcium can reduces LDL-C.for patients with severe stenosis or occlusion of internal carotid artery or middle cerebral artery,high dose atorvastatin calcium treatment can significantly reduce the incidence of cerebrovascular disease,and the longer the time goes on,The more obvious the effect was,the higher the adverse reaction rate of high doseatorvastatin calcium was(14.2%)than that of general dose atorvastatin calcium group(13.3%),but there was no significant difference between the two groups,and there was no serious adverse reaction in both groups.
