Objectives:Objective to evaluate the application value of ultrasonography in patients with papillary thyroid carcinoma,and to analyze the l-ateral cervical lymph node metastasis and its influencing factors in patients with papillary thyroid carcinoma,so as to guide clinical practice.Methods:The experimental analysis of the October 2015-November2017 120 in papillary thyroid carcinoma in our hospital patients,the 120 patients with preoperative thyroid ultrasonography,and markers of lymph nodes,parallel thyroidectomy and neck dissection;postoperative the resection of pathological examination.In accordance with the size of primary tumor(when the primary multiple choice,the largest diameter),divided into three groups,respectively,d < 1.0cm(group A),1.0cm = D = 2.0cm(B group),D2.0cm(group C).The diagnosis of benign and malignant lymph nodes was determined by preoperative ultrasonography,and the accuracy of ultrasonic diagnosis was determined.At the same time,the sex and age of each group,unilateral and bilateral glandular lobectomy,intraoperative tumor invasion and metastasis were analyzed,so as to analyze the relationship between lateral neck metastasis and the above factors.Results:In this study,120 patients,88 patients underwent single neck dissection,32 patients underwent double neck dissection,statistical preoperative lateral cervical lymph ultrasound and postoperative pathological markers corresponding to node 158,preoperative ultrasound suspected there are 68 positive lymph nodes,negative 90;postoperative pathology confirmed positive lymph node 82 76 gold pieces,negative lymph node.Preoperative ultrasonography and postoperative pathology showed that there were 57 positive lymph nodes.Thus,the sensitivity of ultrasound in the diagnosis ofcervical lymph node metastasis of papillary thyroid carcinoma is about69.51%,the specificity is about 85.52%,the positive predictive value is about83.82%,and the negative predictive value is about 72.22%.Ultrasound has a high accuracy in the diagnosis of lateral cervical metastasis of thyroid papillary adenocarcinoma.Age,external invasion of primary foci,bilateral adenocarcinoma.The effect of sex on lateral neck metastasis was not significant.Conclusions: Ultrasonic operation is simple and convenient,and cervical ultrasonography is of great value in the diagnosis of lateral cervical lymph node metastasis of thyroid papillary adenocarcinoma.External invasion of primary tumor is one of the most important risk factors for lateral neck lymph node metastasis.With age increasing,the risk of lateral neck metastasis will increase correspondingly.Bilateral gland lobe cancer has more chance of lateral neck metastasis than unilateral gland lobe.There is no significant correlation between gender and lateral neck metastasis.
