Background: The correlation between serum-ascites albumin gradient(SAAG)and portal pressure in patients with decompensated liver cirrhosis has been confirmed in many clinical studies.SAAG plays an important role in the etiology of cirrhosis and diagnosis to predict esophagogastric varices bleeding,hepatic encephalopathy and hepatorenal syndrome.In the present study,we aim to investigate the relationship between SAAG and serum prealbumin(PA),serum cholinesterase(CHE),prothrombin time(PT),grade of liver function and prognosis in patients with decompensated cirrhosis.Methods: Between January 2015 to March 2017,278 patients with decompensated liver cirrhosis were referred for study in the Second Affiliated Hospital of Chongqing Medical University.Venous blood was collected from all patients for measurement of serum albumin(ALB),globulin(GLB),PA,CHE,total bilirubin(TBIL),PT and serum creatinine(Cr)levels.Simultaneous sampling of the ascites was performed to calculate SAAG.The diameter of the portal vein was measured by abdominal CT.Results:(1)SAAG had positive correlations with portal vein diameter,ascites,CTP classification and the model for end-stage liver disease(MELD)score(P<0.05).(2)SAAG had positive correlations with ALB,PT and CHE(P<0.05),but has no correlations with PA(P=0.931).(3)The differences of SAAG,PA,CHE and PT between Child B and C groups were statistically significant(P<0.05);(4)SAAG,PA,CHE,PT and MELD score could efficaciously reflect the assessment of the liver function and prognosis in patients with decompensated liver cirrhosis(P < 0.01),and SAAG and PT are the protective factors in patients with decompensated cirrhosis.Conclusion: SAAG can reflect effectively and sensitively the degree of portal hypertension,has a close correlation with liver function in decompensated cirrhosis patients.Thus,SAAG has an important value in the liver function classification and prediction prognosis in combination with PA,CHE and PT.
