Objective : To investigate the characteristics of cerebral blood perfusion in Alzheimer's disease(AD)and explore the correlation between cerebral blood perfusion and cognitive function by three-dimension arterial spin labeling(3D ASL).Methods : In this study,3.0T magnetic resonance imaging was performed on 20 elderly patients with AD(AD group)and 20 patients(NC group)who received normal magnetic resonance imaging and 3D-ASL scanning.All patients in the AD group were from hospitalized patients in the Department of Neurology and Rehabilitation Psychiatry of our hospital.The 3D-ASL scan was used to obtain the original images.The original images were imported into the GE MR ADW 4.6 workstation for noise reduction correction.Each of the patients was measured manually using Functool software.The participants were interested in cerebral blood flow(Cerebral blood flow)in the brain area(ROI)and comprehensively assessed the cognitive status of each subject through the neuropsychological scale.We analyzed the cognitive function of the two groups of patients,the characteristics of cerebral blood flow in each region of interest,and the correlation between cerebral blood perfusion and cognitive function.Results: 1.There was no significant difference in gender,age and education level between the two groups(P>0.05).There was a significance difference in Montreal cognitive assessment(Mo CA)scores between the two groups(P=0.001<0.05,t=29.67).There was a statistically significant difference between the two groups in the mini-mental state examination(MMSE)scores(P=0.000<0.05,t=6.32).There was no significant difference in the Hachinski ischemic score(HIS)scores between the two groups(P=0.48>0.05,t=8.32),and vascular dementia was excluded.There was no significant difference in self-rating depression scale(SDS)scores between the two groups(P=0.83>0.05,t=11.55).2.Compared with the NC group,the blood flow of the AD group was lower in the right medial temporal lobe,the right top inferior lobe,the right anterior cingulate cortex,the right posterior cingulate cortex,the left frontal lobe,the left medial temporal lobe,the left anterior cingulate cortex,the left corona radiata,and the left cerebellum,and the difference was significant(P<0.05),especially in the reduction of the bilateral inferior parietal lobe,right Posterior cingulate cortex,and medial left frontal lobe;however,There was no statistically significant difference in CBF between the right frontal lobe,right corona radiata,right cerebellum,and left Posterior cingulate cortex in two groups(P>0.05).3.The results of correlative analysis of cerebral blood flow perfusion and Mo CA scores in multi-site regions of interest in the AD group showed that the perfusion values ? ? of the 9brain regions of interest in the AD group were positively correlated with cognitive scores(r=0.06~0.90,P<0.05),which are: medial right frontal lobe,right inferior lobe,right anterior cingulate gyrus,right posterior cingulate gyrus,right radiant crown,left medial temporal lobe,left anterior cingulate cortex,left posterior cingulate cortex,left cerebellum;however,there was no correlation between blood flow perfusion and cognition scores in the right medial temporal lobe,right cerebellum,medial left frontal lobe,left top inferior lobe,left radiation crown(P>0.05).4.The results of correlative analysis of cerebral blood flow perfusion and MMSE scores in multi-site regions of interest in the AD group showed that the perfusion values ? ? of the 9brain regions of interest in the AD group were positively correlated with cognitive scores(r=0.26~0.78,P<0.05),which are: medial right frontal lobe,right medial temporal lobe,right inferior lobe,right posterior cingulate cortex,right cerebellum,medial left frontal lobe,left medial temporal lobe,left top lobe,left anterior cingulate cortex,left posterior cingulate cortex,left radiation crown.However,there was no correlation between the relative blood flow perfusion and cognitive scores in the right anterior cingulate cortex,right radiation crown,and left cerebellum(P>0.05).Conclusions:(1)Based on magnetic resonance 3D-ASL cerebral perfusion imaging,we can find the hypoperfused region of AD cerebral blood flow,and provide the basis for the diagnosis of early AD,which can be used for the diagnosis of AD.(2)Mo CA and MMSE cognitive score is positively correlated with regional cerebral blood flow in MRI.The CBF of AD can reflect the patient's cognition in some extent.
