Analysis Of The Curative Effect Of Surgical Selection Of 68 Cases Of Giant Cell Tumor Of The Bone Around The Knee

Objective:To investigate the effect of the selection of the surgical procedure of the giant cell tumor of the bone(GCT)around the knee joint on the postoperative recurrence and postoperative function.Methods:From January 2008 to April 2017,68 patients with GCT of knee joint were treated with different surgical methods in Second Clinical Hospital of Shanxi Medical University,confirmed by pathology.And 32 cases male and 36 cases female with an average Of 26.0 years(range from 16 to 59 years)were included.Lesions were located in distal femur in 34 cases,proximal tibia in 34 cases.There were 29 cases were treated by curettage and bone graft,31 cases treated by curettage and bone cement filling,and 8 cases treated by tumor resection and prosthesis reconstruction.Different surgical recurrence rate and the analysis of the function of joint with Enneking functional evaluation system were observed and statistically analyzed.Results:With an average of 34 months(Ranged from 12 months to 77 months),all the patients were followed.The overall recurrence rate were 23.5%.The recurrence rate of bone graft was 31.0%,and curettage with bone cement filling recurrence rate was 22.6%,and the recurrence rate of tumor resection and prosthesis reconstruction was 0%.There is no significant statistically difference between three kinds of surgicaloperations,P=0.207>0.05.The Enneking functional score Of bone graft group?bone cement filling group and prosthetic replacement were 27(25-30)? 26(25-30)and 23(18-29)respectively.It shows no significant statistically difference between bone transplantation filling and bone cement filling,P=0.960>0.05.While there are significant statistically differences between bone transplantation filling and prosthetic replacement?bone cement filling and prosthetic replacement,respectively p=0.01<0.05?P=0.007<0.05.Conclusion:There is no significant difference of recurrence rate among the groups of three kinds of surgical operations.As for the postoperative function,it shows no difference between bone graft and bone cement filling,but there is a significant difference between curettage(including bone graft and bone cement filling)and prosthetic replacement.
