Objective:The incidence of single end point(stroke,myocardial infarction and death)and major complex endpoints were systematically evaluated 30 days after endarterectomy with embolization protector assisted stent implantation and TCD monitoring.Objective evaluation of the advantages and disadvantages of the two operations to guide the clinical treatment decision.Methods:Using the method of computer network database retrieval,in PUBMED /MEDLINE,Central Register of Controlled trials,A randomized controlled study on the efficacy and safety of CEA and CSA was retrieved from the Chinese Biomedical Literature Database and other literature databases between January 2001.0-1 and July12.31,2017.12.31.Cochrane system was used to evaluate the efficacy and safety of CEA and CSA.Independent screening of 2 reviewers to meet inclusion criteria The quality of the literature was evaluated and meta analysis was carried out with Review Manager 5.3software.Results:Finally,7 articles were included in the randomized controlled study,the sample number was 5363 cases.M-H method was used for Meta analysis of the inclusion study,and the results showed that: 1.For severe carotid stenosis,the overall incidence of death and myocardial infarction was similar to that of CSA.There was no significant difference in the incidence of single endpoints: the incidence of stroke within 30 days in the stenting group was higher than that in the OR1.72 95%CI 1.20-2.47 P0.003(including symptomatic carotid stenosis and asymptomatic carotid stenosis;myocardial infarctionwithin 30 days in the endarterectomy group).The birth rate was higher than OR1.7295%CI 0.29-0.81 Pu 0.005.There was no overall mortality rate.Difference.Conclusion:There was no significant difference between CSA and CSA in patients with severe carotid artery stenosis at 30 days after operation(stroke,myocardial infarction and mortality),but there was a significant difference in single endpoint 30 days after operation.
