ObejectiveMetabolic syndrome(MS)is the pathological state of metabolic disorder of protein,fat,carbohydrate and other substances in human body.It is a complex group of metabolic disorders and a harmful factor of cardiovascular and cerebrovascular diseases.This study assessed the epidemiological status of MS in Guanzhou Street community physical examination population in Haizhu District,Guangzhou,and discussion on the influencing factors and health management strategy.It provides scientific guidance for the prevention and management of MS in Guangzhou local community,provides the basis for community health decision-making.Methods(1)Selected 1358 residents(over 6 months old)at Haizhu District Guanzhou Street Community Health Service Center of Guangzhou to participate in physical examination from Jan.to Dec.2016.Collected basic information and laboratory findings(including name,sex,age,type of residence,marital status,occupation,education,lifestyle,lipids,etc).(2)Used SPSS20.0 statistical software for data analysis,from normal distribution data,the measurements are expressed in (?)±S,T test for comparison between two groups,the analysis of variance is used to compare the two groups.Non-normal distribution data are expressed as M(P25,P75),used rank sum test;the counting data is expressed by rate or composition ratio,used?~2 test,used Logistic multivariate regression to analyze the related factors.Calculated ratio(OR)and 95%credibility interval(95%CI),P<0.05 was statistical significance.Results(1)A total of 1358 subjects are included in this study.The average age is(57.63±17.06)years.MS case rate is 12.67%,Male prevalence rate is 12.34%,Female prevalence rate is12.93%,There is no significant difference in the prevalence of MS between men and women(P?0.05);the prevalence of MS in different age groups is significantly different(P<0.05),and the prevalence rate increases with age.(2)There are statistically significant differences in the prevalence rate of MS between different resident types,different marital status,different education level,different occupation type,different medical expense payment way and different life style(P<0.05).(3)Among the components of MS,the prevalence of hypertension is the highest(48.82%),followed by high triglyceride(TG)(39.40%).Among the different combinations of MS components,the prevalence rate of the three component combinations is the highest,and the difference of the prevalence rate among the different combinations is statistically significant(P<0.05),and the prevalence rate decreased with the increase of the number of combinations.(4)Male MS related physical examination and laboratory examination,the MS group compared with the non-MS group,height,weight,waist circumference(WC),lipid accumulation index(LAP),visceral fat index(VAI),body mass index(BMI),fasting blood glucose(FBG),systolic blood pressure(SBP),low density lipoprotein cholesterol(LDL-C),white blood cell count(WBC)difference are statistically significant(P<0.05)and the rest have no statistical significance(P?0.05).Female MS related physical examination and laboratory examination,the MS group compared with the non-MS group,weight,WC,BMI,LAP,SBP,TG,High density lipoprotein cholesterol(HDL-C),LDL-C,hemoglobin(HGB)difference are statistically significant(P<0.05)and the rest have no statistical significance(P?0.05).(5)By logistic regression analysis,age,marital status,occupation,smoking status,WBC,STB,LAP and BMI are the influencing factors of male patients with MS;age,level of education,occupation,method of payment for medical expenses,HGB,LAP,BMI are the influencing factors of female patients with MS.Conclusion(1)Prevalence of MS in Guanzhou Street Community Health examination population in Haizhu District of Guangzhou City in 2016 is at a high level,the prevalence rate of female is higher than that of male,and the prevalence rate gradually increased with the increase of age,suggesting that the medical service mode and technology should be innovated and the suitable MS health management mode should be developed.(2)The prevalence of hypertension is the highest,followed by the prevalence of hypertriglyceridemia.Among the three components of MS,Central obesity+hypertension+hyperglycemia is the highest;among the four components of MS,hypertension+hyperglycemia+central obesity+high TG is the highest.(3)The obesity indexes such as WC,BMI,LAP,VAI etc,are closely related to the incidence of MS in this community,which suggested that in the prevention and treatment of MS,attention should be paid not only to the degree of obesity,but also to the distribution of body fat.(4)Age,marital status,occupation,smoking status,WBC,STB,LAP and BMI are the influencing factors of MS in male health examiners in this community,male examiners in this community age?75,married,serviceman,smoker,WBC,STB and LAP are higher than normal,BMI is lower than normal value or?28(Kg/m~2),are more likely to suffer from MS.(5)Age,level of education,occupation,method of payment for medical expenses,HGB,LAP,BMI are the influencing factors of MS in female health examiners in this community.The older,the more illiterate and semi-illiterate,the person in charge of the state organs and enterprises,and the person who pays all the medical expenses at her own expense,HGB and LAP are higher than normal,BMI is lower than normal value or?28(Kg/m~2),are more likely to suffer from MS in female health examiners in this community.
