Objective: To study the short-term clinical efficacy and safety in patients undergoing percutaneous coronary rotational atherectomy with percutaneous coronary intervention for severe coronary calcification. Methods: A total of 112 patients with coronary artery intervention treatment that confirmed as severe calcification by coronary angiography in Nanjing medical university affiliated GULOU hospital from November 2011 to March 2014 were divided into PTCRA group(50 cases)and non-PTCRA group(62 cases). Both of the two groups were needed to assess for liver function, renal function, blood lipids, color doppler echocardiography and other related inspections after admission to hospital, receive routine treatments of antiplatelet, hypolipidemic, antihypertensive,Beta blockers, hypoglycemic and other therapies before the surgery.Coronary artery calcification and coronary artery diameter reduction of ≥ 70% is the selection and treatment standard both of the two groups. After the interventional therapy, not only the angiographic complications such as the slow-flow/no-reflow phenomenon, perforation, coronary artery dissection,short-term closure and arrhythmia were observed, but also the mortality and occurrence rates of MACE were collectedpostoperative 9-12 months. Results: There is no statistical difference(P > 0.05)between two groups on age,sex,hypertension,smoking,diabetes,cholesterol,high-density lipoprotein cholesterol(hdl-c),low density lipoprotein cholesterol(ldl-c),triglycerides,e GFR,heart ejection fraction(EF)and other factors. And the location of the coronary stenosis,multi-vessel coronary artery diseases, stent length, stent diameter are comparable in the two groups(P > 0.05).The success rate of the two groups is 100%.The mortality and occurrence rates of MACE between the two groups has no statistical difference(P > 0.05);and PTCRA group had less complications(P < 0.05).However,while the Gensini score is ranging from 89.5 to 117.0,the occurrence rate of MACE is significantly lower in the PTCRA group. Conclusion: Percutaneous coronary rotational atherectomy with percutaneous coronary intervention can effectively reduce perioperative complications. The treatment of percutaneous coronary rotational atherectomy for complexity, calcified lesions is safe and effective and has a good short-term clinical efficacy.
