Objective Diabetic nephropathy is a progressive kidney disease.TCM considers that the DN was developed on the deficiency of both qi and yin with "xiaoke",the deficienency of both qi and yin and obstruction of collaterals always runs through the diabetic nephropathy.In recent years,the most of researchers take the DN as an inflammatory disease.NF-?B and MCP-1 play important roles in the pathogenesis of DN inflammation,many researchers believe that diabetic nephropathy renal injury and inflammatory factor process of TCM the evil and blood stasis,kidney injury network ther is a certain relationship.The aim of this study is to observe the change of inflammatory factors before and after the disease process and treatment of DN,and to explore the role of NF-?B and MCP-1 in the pathogenesis of deficienency of both qi and yin,obstruction of collaterals,it means to open a new way for clinical treatment of diabetic nephropathy.Methods Through tail vein injection of STZ and high fat and sugar diet to replicate the early diabetic nephropathy model,set the normal control group(NC),model group(DN),low Chinese medicine dose group(ZD),medium Chinese medicine dose group(ZZ),high Chinese medicine dose group(ZG),and positive control group(DX),with intervening of different doses of tangshen decoction and comparing with valsartan.After 12 weeks,collect the 24h urine to test MAU,Ucr,calculate urinary protein excretion rate and creatinine clearance rate,collect blood to test FBG,CHOL,Scr and TXB2,6-keto-PGF1a,observe the pathological changes from PAS staining of renal tissue,detect the level of NF-?BmRNA in renal tissue through RT-PCR,using Western-blot technology to detect the expression level of MCP-1 protein.Results 1.The changes of rat ordinary circumstances,biochemical index,kidney weight,relative kidney weight,TXA2,PGI2:In DN group,urinary excretion,kidney weight,relative kidney weight,TXA2 is significantly higher than the group ZG and group DX(P<0.05);The Ccr of group DN was significantly higher than the group ZG and group DX(P<0.05);The indicators as BG,CHOL,Ccr between group DN and group ZD are no significant difference.2.The pathologic changes of renal tissue:The pathologic changes of renal tissue in group DN display that mesangial cell proliferation,inflammatory cell infiltration,glomerular atrophy.Compared with group NC,the rats kidney pathology of all the others groups are exacerbated.3.NF-?B,MCP-1 expression level:Compared with group NC,expression of NF-Kb mRNA and MCP-1 protein in the renal tissue are increased significantly in group DN.Compared with group DN,expression of NF-?B mRNA and MCP-1 protein in the renal tissue are increased in group ZG and group DX.Conclusions The deficiency of both qi and yin,obstruction of collaterals were main pathogenesis of early diabetic nephropathy;The expression of NF-?B,MCP-1,and other inflammatory mediators increased in diabetic nephropathy rats;After the intervention of tangshen decoction,the rats'kidney function improved,and the expression of NF-?B,MCP-1 were reduced;In summary,NF-?B,MCP-1 are the material basis of early diabetic nephropathy with the pathogenesis:deficiency of qi and yin,obstruction of collaterals.NF-?B,MCP-1 can be used as new targets for therapy of TCM in DN.
