| Objective: To discuss the clinical value of wave intensity(WI)and high-frequency ultrasonography in evaluation of radial artery wall elasticity and structure in patients of type 2 diabetes mellitus(T2DM).Methods:Atotal of 84 patients of T2 DM were enrolled and divided into two groups:groupB(42 cases,the course ≤10 years)and group C(42 cases,the course >10 years).And 42 healthy subjects were enrolled as controls group(group A).All the diagnosis of T2 DM was conform to the standards set by WHO.Exclusion criteria included heart failure,hypertension,arrhythmiasmoking,alcohol consumptionand chronic renal failure.WI was used to measure the elasticity parameters of radial artery in all the groups.Before the inspection,all inspectors were required to take a rest for 15 minutes,fully exposed the upper limb,using the cuff mercury sphygmomanometer blood pressure measurement of right upper limb 3 times as the WI test standard blood pressure.And then connect the ECG wire in a quiet environment,subjects were asked to take supine position calm breathing,the probe slit as the wrist,After selecting the WI condition,enter the B / M mode,adjust the angle between the beam and the blood flow to 60 °,adjust the direction of the sampling door,so that the two sampling doors accurately placed in the radial artery before and after the outer membrane boundary,real-time tracking radial artery wall motion curve;When the image is stable,press the select key at the same time to collect and store into the WI display screen,enter the blood pressure value,record three groups of WI-related parameters,including the arterial compliance(AC),accelerating wave intensity(W1),decelerating wave intensity(W2),vascular pressure strain elastic modulus(Ep),vascular sclerosis parameters(β),pulse-wave velocity(PWVβ)and the negative area(NA).High-frequency ultrasonography was used to measure the intima medial thickness of both sides of the radial artery(rIMT).The results were analyzed statistically.Results:1.The general parameters in each group:There were no significant differences in age,sex ratio,heart rate,systolic blood pressure and diastolic blood pressure between groups A,B and C(all P> 0.05).2.The WI parameters of radial artery in each group:AC,W1 andW2 in group B and C were significantly lower than those in group A(all P<0.05).AC,W1 and W2 in group C were significantly lower than those in group B(all P<0.05).Ep,β and PWVβ in group B and C were significantly higher than those in group A(all P<0.05).Ep,β,PWVβ in group C were significantly higher than those in group B(all P<0.05).There was no statistical difference of NA among 3 groups(all P>0.05).3.Comparison of radial artery structure and rIMT in each group:(1)In group A,the intima of bilateral radial artery was smooth and the tube wall showed two parallel linear echoes in longitudinal section.In group B and C,the thickness of bilateral radial artery was thickened,not smooth,wall thickness is uneven.(2)The rIMT in group B and C were significantly higher than that in group A(all P<0.05)and the rIMT in group C was higher than that in group B(P<0.05).Conclusion: The elasticity and structure of radial artery in T2 DM patients with varying degrees of damage has been damaged differently.WI can evaluate the abnormal changes of bilateral radial artery elasticity in T2 DM patients quantitatively and accurately.High frequency ultrasound can show the structural changes of bilateral radial artery in T2 DM patients clearly and intuitively.The combination of the two can provide an accurate and objective basis for clinical diagnosis,and has important clinical significance in the clinical treatment and complications prevention of vascular lesions in T2 DM patients. |
