Objective:Bronchial astnma(astnma)and chronic obstructive pulmonary disease(COPD)are the two high incidence airway diseases of respiratory system,although each one has a difference,along the progressing of the scientific research,people have begun to gradually realize that some of patients with the clinical characteristics of COPD and asthma at the same time,the Asthma and COPD Overlap Syndrome(ACOS).However the lack of relevant guideline of the particular disease,so it is great academic value of in depth study.Eosinophils(EOS),as a kind of immune cells,with a variety of functions,participate in a variety of reactions and developments,and the cells components in the sputum can directly response the airway inflammation.The purpose of this paper is to study the expression and meaning of eosinophil counted by inducing sputum in the patients with acute exacerbation of Asthma and COPD overlap syndrome.Methods:We analysed datas from 32 cases with acute exacerbation of COPD,31 cases with acute bronchial asthma and 22 cases with acute exacerbation of Asthma and COPD Overlap Syndrome.All 85 patients were in accordance with the inclusive criteria,and hospitalized in the First Affi;iated Hospital of Dalian Medical University from June 2016 to January2017.The items of each case we collected including sex,age,body mass index(BMI),smoking history,allergic history,degree of education,the proportion of eosinophils in the sputum,the count of peripheral blood eosinophils cells,the count of peripheral blood white cells,procalcitonin,total serum Ig E and pulmonary functiontest.Measurement data were given as means±SD,if obey normal distribution,the comparison between groups by one-way analysis of variance,correlations by pearson correlation analysis.IF not,the comparison between groups by rank test,correlation with spearman correlation analysis.The enumeration data can be described by constituent ratio and compared by chi-square.Results:1.The average of sputum eosinophil count in the group of Asthma and COPD Overlap Syndrome was 4.14±5.15%,was higher than the group of Asthma's 3.23±3.33%and the group of COPD's 0.40±0.80%,and there were statistically significant differences between the three groups,the group of Asthma and COPD Overlap Syndrome and the group of Asthma were higher than the group of COPD,all P<0.01.2.The level of total serum Ig E in the group of Athma was higher than the group of COPD,P<0.01.3.The improvement value and improvement rate of FEV1 in the group of Asthma and COPD Overlap Syndrome were significantly higher than the group of Asthma and COPD,the improvement value and improvement rate of FEV1 in the group of Asthma were significantly higher than the group of COPD,all P<0.01.4.The cultural degree in the group of Asthma and COPD Overlap Syndrome was higher than the group of Asthma(P=0.002<0.01)and the group of COPD(P=0.006<0.01).Conclusions:1.The sputum eosinophil level was increased in the patients with acute exacerbation of the Asthma and COPD overlap syndrome,there was statistically difference between in the patients with COPD acute exacerbation,it can be used as the different diagnosis indicators of the two.The sputum eosinophil count also can be used to differentiate the acute attack of Asthma and COPD acute exacerbation,but it can't be used to differentiate the exacerbation of Asthma and COPD overlap syndrome and the acute attack of Asthma.2.The level of total serum Ig E in the patiens with the acute attack of Asthma washigher than the pations with the acute attack of COPD.3.The improvement value and improvement rate of FEV1 in the group of patients with Asthma and COPD overlap syndrome were higher than the patiens with the acute attack of Asthma and the patiens with the acute exacerbation of COPD.
