Objective: To assess the clinical effects of Hai-ao bone graft combined with Hai-ao collagen membrane in peri-implant bone augmentation and bone defect reconstruction,provide guidance for clinical work.Methods:15 bone defects patients including delayed implant(5),immediate implant(3)maxillary sinus elevation(4)and mandibular cysts(3)were selected.Conventional surgery area disinfection and draping was completd Preoperativly.The incision was made at the surgical area after local infiltration anesthesia.Buccal mucoperiosteal was reflected in a full thickness approach.Implantation was initiated at the implant site with a three edged drill,a 2.0mm twist drill was then advanced to a depth that was measured from the preoperative radiograph.Gradually enlarged the bone,then place the implants.The Hai-ao bone graft was used to fill the bone defects and the cavity beneath the elevated sinus membrane in the implant surgery and also used as fillings of cyst cavity,and the bone graft surface covered with Hai-ao collagen membranes.Whether place the impiant according to bone density and bone mass.If the final torque of implant is more than 30 Ncm,it will be screwed into the healing abutment for non-submerged healing,conversely,it will be screwed into the cover screw for submerged healing.All patients were followed up postoperatively after the first week,3 months,and 6 months.The clinical evaluation focus on the osseointegration of implants,the stability of the implants prosthesis and the healing of flap around implants,X-ray examinations were taken immediately after operation as well as postoperatively after 3 and 6 months.Radiographic evaluation focused on the bone remodeling in surgical area.Results:There were 13 patients felt well 7 days after surgery,the mucosal with normal color and no edema.One case of maxillary sinus lifting patient had orbital swelling and nasal bleeding,and recovered 15 days later.There was only one immediate implant patient occurred wound dehiscence after the surgery,but with anti-inflammatory,cleaning wound and drug therapy,the wound healed well.Other patients were all primary healing.In the period of 6 months follow-up,all implants were clinically stable and the early survival rate is 100%,13 patients have completed denture installation and were satisfied with implant denture.Compared with preoperative X-ray,all implants have good osseointegration and new bone around the implants,The sinus and cyst cavity were filled with new bone,but the bone mineral density in sinus and cyst cavit was lower than the normal bone after three months,and there is a clear boundary between the new bone and normal bone.While,after six months,the bone mineral density was higher than the 3 months ago,the boundary between the new bone and normal bone isn't clear and the trabecular bone appeared in the grafted area,but the bone mineral density was uneven.Conclusions: This clinical observation confirmed the Hai-ao bone graft combined with Hai-ao collagen membrane can not only repair the small range,but also plays a role in large bone defects regeneration.Therefore,Hai-ao bone graft is a osteoeonduetive bone graft,Hai-ao collagen membrane as barrier membrane can effectively prevent connective tissue develop in to the defect region and product fibrous tissue.
