| Background and objectiveInflammatory bowel disease(IBD)is a kind of autoimmune disease.Its extra intestinal manifestations can be involved in various systems.This paper through a clinical study to investigate incidence and clinical features related to mainly mucocutaneous disease including recurrent aphthous oral ulcers(RAU),erythema nodosum(EN),gangrenous pyoderma(PG),psoriasis of inflammatory bowel disease.We wish through this study we can find the relationship between intestinal tract and mucocutaneous manifestions,providing the theory for early diagnosis of IBD.MethodsConsecutive IBD patients were enrolled between March 2016 and January 2017.Clinical data of IBD patients included,mucocutaneous manifestations,the Montreal phenotypes,disease activity(Crohn’s disease activity index[CDAI]in Crohn’s disease[CD],Mayo score in ulcerative colitis[UC]),and laboratory tests-.Every patient underwent a detailed physical examination by both gastroenterologists and dermatologists.Results1.A total of 401 IBD patients were included in our study consisting of 307 CD patients and 94 UC patients.2.In our study,a total of 84 patients with IBD had mucocutaneous diseases.The incidence in CD and UC was 24.10%(74/307 and 10.64%(10/94,10.64%).The most common mucocutaneous manifestations in IBD were RAU(63,15.96%).Specially,the prevalences of EN,psoriasis and PG were(13,3.24%),(10,2.49%)and(1,0.25%).There were 3 patients with both psoriasis and RAU.3.We observed that RAU(CD/UC,18.89%/5.32%,p=0.002)was more frequently seen in CD than in UC.11 cases of EN appeared in the active phase of IBD,two cases appeared in the remission period,EN has a close relationship with disease activity in our study(P=0.026).9 cases of female patients with erythema nodosum,male 4 cases,women were significantly higher than men(p=0.036).There was only one CD patient with PG.What’s more,the prevelance of psoriasis in IBD was 2.49%(10 patients)4.Multiple logistic regression identified the following risk factors for oning mucocutaneous manifestions in IBD:IBD type(odds ratio(OR)=2.419,95%confidence interval(CI)=1.166-5.018,p=0.018)and joint pain.(odds ratio(OR)=0.219,95%confidence interval(CI)=0.121-0.398,p=0.000).The prevalence of mucocutaneous manifestions in IBD patients with two risk factors was significantly higher than that in IBD patients without risk factors.ConclusionRAU,EN and psoriasis were the most common mucocutaneous manifestations related to IBD.IBD type and joint pain are associated with ongoing mucocutaneous manifetations in IBD patients. |
