Research On The Correlation Between Serum Tumor Markers And Non-small Cell Lung Cancer Classification,Staging And Chemotherap

Objective:Based on the the pathological diagnosis of non-small cell lung cancer(NSCLC)in Affiliated Hospital of Yan'an University,the dynamic evolution of serum tumor marker(TM)known as carcinoembryonic antigen(CEA),neuron specific enolase(NSE),cytokeratin 19 fragment(Cyfra21-1),carbohydrate antigen 125(CA125)levels were studied to hope to improve the clinicians' knowledge on the classification,staging,curative effect evaluation and prognosis evaluation of NSCLC diagnosis.Methods:The complete data of 92 patients with NSCLC and received chemotherapy from January 2011 to January 2014 was collected,including the TM level before chemotherapy and 2 courses after chemotherapy,imaging data and overall survival(OS).The relationship between 4-TM with NSCLC patients' gender,age,pathology,stage,chemotherapy efficacy and prognosis was analyzed.SPSS19.0 software was used for statistical analysis,the two groups were compared by using independent t-test,single factor analysis of variance(ANOVA)was used in the comparison among the groups,and SNK-q test was used in the comparison between two groups.The positive rate of count data was inspected by using X2 test,Kaplan-Meier method was used to calculate OS and draw survival curves,and Log-rank analysis was used to tested and analyzed the correlation between 4-TM and OS.Statistical results P <0.05 was considered statistically significant,P<0.01 was considered significant statistical significance.Results:1.The expression level and positive rate of Serum CEA and CA125 in adenocarcinoma were higher than the squamous cell carcinoma patients,the difference was significant on statistics(P<0.05).The expression level and positive rate of serum Cyfra21-1 was higher than adenocarcinoma,the difference was significant on statistics(P<0.05);In clinical staging of TNM,the expression level and positive rate of IV CEA was higher than that with II and III,the differences were significant on statistics(P<0.05),the positive rate of IV CA125 patients was higher than that of II and III,the difference was significant on statistics(P<0.01).2.The level of serum CEA and CA125 before chemotherapy of invalid group were higher than the level of valid group,the difference was statistically significant(P< 0.01).3.After chemotherapy,the 4-TM level decreased obviously,the difference was significant on statistics(P<0.01).The level of CA125 and Cyfra21-1 after chemotherapy in invalid group were higher than those before chemotherapy,the differences were significant on statistics(P<0.05).4.The level of CEA before chemotherapy was negatively correlated with OS,and the level of serum CEA in 1 years was significantly higher than that in 2 years and 3 years,and the difference was significant on statistics(P<0.05).The OS of negative group in Serum CEA and CA125 was higher than the positive group,the difference was significant on statistics(P<0.01).Conclusion:1.For the patients with NSCLC,the concentration level and positive rate of CEA,CA125 and Cyfra21-1 levels before chemotherapy can contribute to the pathological classification and diagnosis.The concentration of CEA and CA125 were helpful to the clinical stage of TNM.2.For the patients with NSCLC,the level of serum CEA and CA125 before chemotherapy can be used as a good predictor of short-term efficacy of chemotherapy.The level of serum 4-TM decreased significantly after chemotherapy suggested that the short-term curative effect was good.The level of serum CA125 and Cyfra21-1 increased significantly after chemotherapy suggested that the short-term curative effect was poor.3.For the patients with NSCLC,the negative group of CEA and CA125 before chemotherapy predicted a longer OS.
