ObjectiveTo investigate the statistical differences of the UHRF1 gene(ubiquitin-like with PHD and ring finger domains 1,ubiquitin like PHD and ring finger domain containing 1)in benign and malignant pleural effusion,to find the gene marker helpful for the differential diagnosis of benign and malignant pleural effusion and applied in the differential diagnosis and treatment of malignant pleural effusion.MethodsFrom October,2015 to February,2017,60 cases of patients with lung cancer and malignant pleural effusion(39 cases of adenocarcinoma,7 cases of squamous cell carcinoma,14 cases of small cell lung cancer)and 64 cases of patients with benign pleural effusion(36 cases of tuberculous pleural effusion,21 cases of other inflammatory pleural effusion,2 cases of emphysema,5 cases of chronic obstructive emphysema)in Jiangning Hospital Affiliated to Nanjing Medical University were selected,Total RNA was extracted from all samples and reversely transcribed to c-DNA.Real-time fluorescence quantitative PCR technique(SYBR Green I by reverse transcription method)was applied to detect the expression level of UHRFI mRNA in exfoliated cell of pleural effusion,to analyse the differences of expression between benign and malignant pleural effusion,and to analyse the relation between UHRF1 mRNA and clinicopathological parameters in lung cancer patients.2-??CT was used to calculate the relative expression of UHRF1,the receiver operating characteristic curve(receiver operating,characteristic,ROC)and the area under the curve(area under the ROC curve,AUC)were applied to estimate the sensitivity and specificity of the expression of UHRF1 mRNA in the diagnosis of malignant pleural effusion.The expression of UHRF1 was respectively compared with CEA of pleural effussion,exfoliative cytological examination of pleural effusion and the detection of DNA heteroploid cell.SPSS20.0 statistical software was used,t test was applied in measurement data,and One-Way ANOVA analysis was applied in data among multiple groups.ResultsThe relative expression of UHRF1 mRNA in the malignant pleural effusion group was(4.53+4.33),which was significantly higher than that in the benign pleural effusion group(1.61+1.56),the difference was statistically significant(t=5.057,P=0.000).The receiver operating characteristic curve analysis showed that the area of UHRF1 mRNA,CEA detection and combined detection under the curve were0.753,0.879,0.861,and the sensitivity was 53.48%;,78.18%,74.42%,and the specificity was 88.24%,93.44%,86.27%respectively.The expression levels of UHRF1 mRNA were independent of sex,age,histological type,tumor location,and smoking history(P>0.05),and were not correlated with CEA levels,which were related to exfoliated cells and DNA aneuploid cells.ConclusionThe results showed that the relative expression of UHRF1 gene in malignant pleural effusion was significantly higher than that in benign pleural effusion,and the relative expression of UHRF1 gene in malignant pleural effusion where cytological diagnosis basis was found was signifieantly higher than that in malignant pleural effusion where cytological diagnosis basis was not found.These provided new ideas and methods for the diagnosis and differential diagnosis of clinical pleural effusion.UHRF1 was expected to become a new molecular marker which worked for the differential diagnosis of benign and malignant pleural effusion.And the expression of UHRFlgene could provide the basis for the diagnosis and treatment of malignant pleural effusion and lung cancer.
